潜伏期硬膜外分娩镇痛对产妇及新生儿结局影响

Effect of the Incubation Period of Epidural Labor Analgesia of Maternal and Neonatal Outcome

目的:分析潜伏期硬膜外分娩镇痛对产妇与新生儿结局的影响。方法:收集笔者所在医院2013年6月-2014年6月期间诊治的240例初产妇作为研究对象,将其中的80例宫口<3 cm的潜伏期并实施硬膜外阻滞分娩镇痛的产妇设为甲组,将其中的80例宫口>3 cm的活跃期并实施硬膜外阻滞分娩镇痛的产妇设为乙组,另外80例未实施硬膜外阻滞分娩镇痛的产妇设为对照组,对三组产妇与新生儿结局进行对比分析。结果:三组产妇组间血流动力学比较,差异无统计学意义(P>0.05);甲组产妇和乙组产妇的第二产程时间均显著长于对照组(P<0.05),第一产程、第三产程以及出血量比较差异均无统计学意义(P>0.05);甲组产妇和乙组产妇镇痛效果和新生儿Apgar评分比较差异均无统计学意义(P>0.05)。结论:在产妇潜伏期的时候开展硬膜外分娩镇痛可显著提高镇痛效果,改善母婴结局,且有较高的安全性,有非常重要的临床价值,值得临床推广应用。

Objective： To analyze the effect of the incubation period of epidural labor analgesia of maternal and neonatal’ outcome.Method：240 primiparas in our hospital from June 2013 to June 2014 were collected as the study objects. 80 primiparas in the incubation period with cervix less than 3 cm and implemented with epidural labor analgesia was group A; 80 primiparas in the active period with cervix greater than 3 cm and implemented with epidural labor analgesia was group B; other 80 primiparas without implemented epidural labor analgesia was the control group. Primiparas and neonatal’ outcome were compared and analyzed of the three groups.Result： Three groups’ hemodynamic were compared, there were no statisticlly significant diffreence of the three groups（P〉0.05）;Group A and group B’s second stage of labor were both obvious longer than the control group（P〈0.05）and there was no statistical significance in the comparison of the first stage labor, the third labor stage and the blood bleeding（P〉0.05）; There was no statisticlly significancant diffreence between the group A and group B’s analgesic effect（P〉0.05）; The Apgar scores of group A and group B’s neonatal ,there was no obvious statisticlly significant difference with the control group（P〉0.05）.Conclusion： Conducting epidural labor analgesia in the incubation period can significantly improve the analgesic effect and maternal and neonatal outcomes,and has high security and important clinical value, which is worth promoting further in the clinical application.
