肋骨环抱器与肋骨接骨板在肋骨内固定术中的应用比较

Comparison of Grab Holder and Plate Fixator in Rib Internal Fixation Operation after Severe Thoracic Trauma

目的:比较肋骨环抱器和肋骨接骨板在创伤后肋骨内固定手术中使用的优缺点。方法:通过总结我科2010年1月至2014年6月实施的182例肋骨内固定手术,在应用指证、操作简易度、手术切口、固定效果、愈合效果等多方面比较两者的优缺点。结果:肋骨环抱器适用范围大,操作更简单,但影响血供和骨质愈合;肋骨接骨板可用于肋骨骨质破碎或缺失,不影响血供和骨质愈合,但手术需要特殊器械,价格较贵。结论:合理选择不同的固定材料,或者联合应用两种材料,可以达到最佳手术效果。

Objective： Tocompare the usage of Ti-Ni shape memory grab holder and plate fixator in rib internal fixation operation after severe trauma.Methods： 182 cases of rib internal fixation operation were performed during January 2010 to June 2014 in our department.These two fixation devices were compared in implant indication,operative difficulty,operation incision,fixation effect,healing effect and other aspects.Results： The embracing fixator can be used in more area of ribs,the operation is more simple,but it will destruct blood supply and affect the fracture healing;The plate with screws can be used in comminuted rib fractures or ribs missing.It does not affect the blood supply and bone healing,but the operation requires special equipments and the price is more expensive.Conclusion： To choose the different fixation materials correctly,or combined use of these two kinds,can achieve the best effect of operation.
