右室间隔部起搏和右室心尖部起搏对心脏舒张功能的影响 被引量:4

Effects of Different Septal Pacing of Right Ventricle on Left Ventricular Diastolic Functions

摘要 目的:通过超声心动图评价右室间隔起搏和右室心尖部起搏对患者心脏舒张功能的影响。方法:选取2010年1月到2013年1月于我院行起搏器治疗的102例患者,按照起搏部位的不同分为右心室间隔起搏组(RVS组,n=43例)和右心室心尖部起搏组(RVA组,n=59例),分别对所有患者术前、术后1个月、术后1年的左心室等容舒张时间(left ventricular isovolumic relaxation time,IVRT),二尖瓣舒张早期速度(the blood now velocitv of mitralannulus during early diastole,E)与舒张晚期速度(the blood now velocity of mitralannulus during atrial contraction,A)的比值即E/A比值,以及Em/Am比值进行检测以评估患者的心脏舒张功能。结果:所有患者均完成随访,术后1个月随访两组患者的心脏舒张功能评价没有明显统计学差异;术后1年的心脏舒张功能评价RVS组优于RVA组。结论:右室间隔起搏对于患者心脏舒张功能的影响低于右室心尖部起搏,可能为起搏器植入患者理想的起搏部位。 Objective: To evaluate the effect of different right ventricular septal pacing on the left ventricular diastolic function.Methods: We randomly enrolled 102 patients who implanted pacemaker in Cangzhou Central Hospital from January 2010 to January 2013,and devide them into the right ventrcular septum group(n=43) and right ventricular apical pacing group(n=59).All patients' left ventricular isovolumic relaxation time( IVRT) and the blood now velocitv of mitralannulus during early diastole(E),the blood now velocity of mitralannulus during atrial contraction(A),and E/A ratio,and Em/Am ratio were tested to assess the patient's cardiac diastolic function before operation,after one month,and after one year.Results: All patients completed one year follow-up,after one month follow-up the evaluation of cardiac diastolic function has no significant difference;after one year,the RVS group's evaluation of diastolic function was better than the RVA group.Conclusion: Patients with right ventricular septal pacing has less infection on cardiac diastolic function than patients with right ventricular apical pacing,May be the pacing site ideal for patients.
