动态增强MRI在垂体微腺瘤诊断中的应用 被引量:4

摘要 目的 探讨了垂体动态增强扫描对垂体微腺瘤诊断价值。方法 选择2013年1月至2014年1月间我院疑诊为垂体微腺瘤患者88例为研究对象,所有患者均于入院后2d内行鞍区MR平扫、动态增强扫描及延迟扫描。以手术病理及临床证实结果为标准诊断,观察并比较MR平扫、动态增强扫描及延迟扫描对垂体微腺瘤诊断情况。结果 经病理或临床证实为垂体微腺瘤56例,均为单发,其中位于垂体前叶49例,左侧27例,右侧29例。长径2-10mm,平均5.3±2.2mm。MR平扫显示垂体内异常信号28例,动态增强扫描显示病灶55例,延迟增强扫描显示病灶32例。动态增强扫描对垂体微腺瘤显示率显著高于平扫及延迟增强,差异具有统计学意义(P〈0.05)。动态增强扫描对垂体微腺瘤诊断敏感性、特异性、阳性预测率及阴性预测率分别为94.6%、93.7%、96.4%、90.9%。结论 垂体动态增强扫描对垂体微腺瘤诊断价值高,值得临床推广应用。 Objective To investigate the diagnosis value of dynamic enhanced scan of the pituitary pituitary micro adenoma. Methods 88 patients with suspected pituitary micro adenoma in our hospital were chosen as the research object, All patients underwent admission plain scan, dynamic enhanced scan and delayed scan within 2 days after admission. With operation and pathology confirmed the results for the standard diagnosis,To observe and compare the MR plain scan, dynamic enhanced scan and delayed scan of the pituitary micro adenoma diagnosis. Results As proved by pathology or clinical pituitary micro adenoma in 56 cases,Which was located in the anterior pituitary in 49 cases, 27 cases on the left side, the right side in 29 cases. Length to diameter 2--10mm, average 5. 3+2.2mm. MR scan showed abnormal signal intensity in 28 cases of pituitary,Dynamic enhanced scan showed the lesions in 55 cases, Delayed enhancement showed the le- sions of 32 cases. Dynamic enhanced scanning of pituitary micro adenoma showed significantly higher rates of plain scan and delayed enhancement, there were statistically significant differences (P〈0. 05). The diagnostic sensitivity, specificity, positive predictive rate and negative predictive rate of Dynamic enhanced scanning of the pituitary micro adenoma were 94. 6% ,93. 7%, 96. 4%, 90. 9%. Conclusion Pituitary dynamic enhanced scan has high diagnosis value in pituitary micro adenoma, worthy of clinical application.
