高血压脑出血短期预后相关影响因素分折

Analysis of related factors of short-term prognosis of hypertensive cerebral hemorrhage

目的分析影响高血压脑出血短期预后的相关因素。方法选择2013年10月一2014年10月收治的80例高血压脑出血患者作为研究对象，所有患者均于发病6h内入院，动态监测患者血压变化情况。统计患者收缩压、舒张压、出血部位、出血量、性别、年龄等资料，根据患者神经功能缺损评分得分情况分为好转组22例与未好转组58例，分析影响高血压脑出血患者短期预后的相关因素。计数资料采用χ2检验，计量资料采用t检验，采用多因素Logistic回归分析高血压脑出血短期预后的影响因素，P〈0．05为差异有统计学意义。结果年龄（OR=5．647，95％CI为2．621～12．166）、格拉斯哥昏迷指数（Glasgow coma scale，GCS）得分（OR=0．322，95％CI为0．101～0．796）是影响高血压脑出血患者短期预后的独立影响因素。结论高龄、GCS得分低是导致高血压脑出血患者短期预后不良的独立危险因素。为改善患者预后，必须重视对高龄、意识状态较差脑出血患者的治疗与评估，确立有效的诊疗方案，以优化其生存质量。

Objective To analyze the related factors of short-term prognosis of hypertensive cerebral hemorrhage.Methods 80 patients with hypertensive cerebral hemorrhage admitted from October 2013 to October 2014 were selected.All of the patients were admitted within 6 h after paroxysm.The changes of blood pressure were dynamically detected.The systolic blood pressure,di- astolic blood pressure,bleeding parts,amount of bleeding,gender, age and other information in patients were counted up.According to the nerve function defect score of the patients,all the patients were divided into better outcome group（n=22） and deterioration group（n=58）.The related factors influencing short-term prognosis of patients with hypertensive cerebral hemorrhage were analyzed. The count data was processed by chi square test,measurement data by t test,the factors of short-term prognosis of hypertensive cere- bral hemorrhage were analyzed by Logistic analysis,P〈0.05 was considered statistically significant.Results Age（OR=5.647,95% CI 2.621-12.166） and Glasgow coma scale（GCS） score（OR=0.322,95%Cl 0.101-0.796） were the independent factors influencing the short-term prognosis of patients with hypertensive cerebral hemorrhage.Conclusion Aged and low GCS score are the inde- pendent risk factors of short-term adverseness in patients with hypertensive cerebral hemorrhage.In order to improve the progno- sis,docters should pay attention to the treatment and evaluation of elderly and poor state of consciousness patients;establish an ef- feetive treatment program,optimizing the patient＇s quality of life.
