自闭症并听力损失患儿的人工耳蜗植入 被引量:2

摘要 目的观察并评价自闭症合并听力损失患者的人工耳蜗植入效果及自闭症康复效果,探讨该类患者人工耳蜗植入的可能性。方法观察两例自闭症患者的手术植入、术后电极阻抗、神经遥测反应波形及耳蜗位平片,利用神经反应遥测技术结合行为测听法对患者进行术后人工耳蜗调试,采用《婴幼儿有意义听觉整合量表》(Infant-toddler Meaningful Auditory Integration Scale,IT-MAIS)评估患者听觉言语康复效果,《克氏孤独症行为量表》评估自闭症的康复效果。结果两例自闭症患者手术顺利,植入电极阻抗在正常范围,经人工耳蜗调试可获得较好的助听效果,自闭症的症状有改善。结论自闭症合并听力损失患者植入人工耳蜗后有较好的助听效果,有助于自闭症症状的改善,但其植入适应证的选择仍需谨慎。 Objective To observe and evaluate the rehabilitation outcome of children with autism spectrum disorder and hearing loss after cochlear implantation and to explore the effectiveness of cochlear implantation in children suffering from both autism and severe hearing loss. Methods Cochlear implantation was performed to two children with autism and hearing loss. Postoperative electrode impedance, neural response telemetry (NRT) thresholds and cochlear plain film were observed. NRT thresholds and behavior T-levels were tested during the mapping procedure. Infant-toddler Meaningful Auditory Integration Scale (IT-MAIS)was used to evaluate the auditory and speech rehabilitation outcome of the children. Clancy Autism Behavior Scale(CABS) was also used to assess the improvements of autism disorders after cochlear implantation. Results The cochlear implantation were successful in the 2 children with autism. The impedance of the insertion electrodes was normal. After mapping of cochlear implants, the children obtained good hearing results and the symptoms of autism were also improved. Conclusion Cochlear implantation was proved effective for children with autism spectrum disorder and hearing loss, but the choice of indications for cochlear implantation still needs to be careful.
