慢性阻塞性肺疾病合并呼吸衰竭应用无创正压通气临床疗效

摘要 目的探讨慢性阻塞性肺疾病合并呼吸衰竭应用无创正压通气治疗的临床疗效。方法 80例慢性阻塞性肺疾病合并呼吸衰竭患者,随机分为观察组和对照组,每组40例,对照组患者应用常规治疗,观察组在对照组治疗的基础上给予无创正压通气治疗,对比分析两组患者治疗前后呼吸频率、心率、动脉血氧分压(Pa O2)、动脉血二氧化碳分压(Pa CO2)、p H值。结果治疗前两组患者心率、呼吸频率、Pa CO2、Pa O2、p H值比较差异无统计学意义(P>0.05),治疗后观察组患者心率、呼吸频率、Pa CO2、Pa O2、p H值均明显优于对照组(P<0.05)。结论无创正压通气治疗慢性阻塞性肺疾病合并呼吸衰竭疗效确切,可有效的改善患者的临床症状,阻止病情发展,有助于呼吸衰竭的纠正,值得临床推广和应用。 Objective To investigate clinical effect of non-invasive positive pressure ventilation for chronic obstructive pulmonary disease combined with respiratory failure. Methods A total of 80 chronic obstructive pulmonary disease combined with respiratory failure patients were randomly divided into observation group and control group, with 40 cases in each group. The control group received conventional treatment, and the observation group received additional non-invasive positive pressure ventilation. Comparison and analysis were made on respiratory rate, heart rate, arterial partial pressure of oxygen (PaO2), arterial partial pressure of carbon dioxide (PaCO2), and pH in the two groups before and after treatment. Results There was no statistically significant difference of heart rate, respiratory rate, PaCO2, PaO2, and pH between the two groups before treatment (P〉0.05). After treatment, the observation group had obviously better heart rate, respiratory rate, PaCO2, PaO2, and pH than the control group (P〈0.05). Conclusion Non-invasive positive pressure ventilation provides precise effect in treating chronic obstructive pulmonary disease combined with respiratory failure. It can effectively improve clinical symptoms, prevent progression of disease, and correct respiratory failure. This method is worthy of clinical promotion and application.
