乳腺纤维腺瘤复发为叶状肿瘤3例临床病理观察

Transformation of fibroadenoma into phyllodes tumours after recurrence: a clinicopathologic analysis of three cases

目的探讨纤维腺瘤与叶状肿瘤的病理鉴别诊断。方法对3例纤维腺瘤复发后病例进行临床病史和组织学复习并复习文献。结果 3例首次活检时均诊断为纤维腺瘤,其中1例复发后诊断为叶状肿瘤,1例复发后诊断为纤维腺瘤伴早期叶状肿瘤改变,1例第1次复发后仍诊断纤维腺瘤,但第2次复发后诊断为叶状肿瘤。结论纤维腺瘤与叶状肿瘤的鉴别要紧密结合临床和影像学,并掌握早期叶状肿瘤的几个组织学特点。叶状肿瘤的发病年龄不仅仅局限于中老年女性,年轻女性也有叶状肿瘤的发生。

To discuss the differential diagnosis between fibroadenoma and phyllodes tumours. Methods The medical history and histological changes were reviewed in three women of breast fibroadenoma who recurred once or more times after mammary biopsy and the diagnosis was again discussed. Results Three women including two young women were diagnosed as fibroadenoma after mammary biopsy when they first come to hospital; but after one year to five years, recurrence happened to them, once or even more times. The last diagnosis was phyllodes tumours. Conclusion The differential diagnosis between fibroadenoma and phyllodes turnouts can be made with close combination of clinical and image features, and being familiar with the histological characteristics of early phyllodes tumours. Phyllodes tumours not only occur in middle -aged women, but also in young lady.
