喉罩在气管切开术中的运用

摘要 目的观察喉罩用于危重患者经皮气管切开术(PT)的临床效果。方法选取该院2013年3月至2015年2月收治的成人颅脑损伤术后需行PT患者38例,分为喉罩组(LMA组)和气管插管组(ETT组),每组19例,记录两组患者行PT前置入支气管镜3 min(T1)、PT时(T2)、PT完成后3 min(T3)的动脉血氧分压(Pa O2)和动脉血二氧化碳分压(Pa CO2)值,气管内结构可视化程度分级患者数,通气质量及相关不良反应。结果两组患者行PT期间各时点Pa O2均有下降,Pa CO2均有上升,但ETT组变化程度明显高于LMA组,差异均有统计学意义(P<0.05);气管内结构可视化程度,LMA组良好率为73.7%(14/19),明显高于ETT组的42.0%(8/19),差异有统计学意义(P<0.05);ETT组中3例发生气管导管滑脱,1例气管后壁划伤,重新ETT行PT,而LMA组中仅1例置入喉罩失败,行ETT后顺利完成手术。LMA组行PT过程中不良反应发生率低于ETT组,差异有统计学意义(P<0.05)。结论与ETT相比,LMA在气管内结构的可视化程度及避免因气管导管滑脱发生缺氧危险等方面均具有明显优势,清晰的视野不仅降低了严重并发症的发生率,而且提高了麻醉安全性。
