窄带成像技术对结肠息肉病理诊断的预测价值 被引量:2

Efficiency of NBI Colonoscopy in the Diagnosis of Colorectal Polyps Lesions

摘要 目的评价窄带成像技术(NBI)的应用对结肠息肉的诊断价值。方法对2013年8月-2014年8月经天津市咸水沽医院结肠镜检出的69例结肠息肉患者使用窄带成像技术观察息肉表面血管形态(CP),对病理分型进行预测,将息肉切除后的标本进行病理学检查,研究表面血管形态与病理学的关系。结果 69例患者中有163处病变,依据病理诊断结果 ,共有93处肿瘤性病变,70处非肿瘤性病变,普通NBI观察结肠隆起性病变时,敏感性为93.50%,特异性为81.40%,准确度为88.34%。而直径<5mm的病变,敏感性为94.40%,特异性为96.00%,准确度为95.19%;同时,直径>5mm的病变,敏感性为92.30%,特异性为45.00%,准确度为76.27%。结论 NBI模式下观察结肠隆起性病变的CP形态对预测其病理分型,即鉴别其为肿瘤性或非肿瘤性病变具有较为可靠的价值。 Objective To evaluate the value of narrow band imaging(NBI) in diagnosis of colonic polyps in the application. Methods From 2013 August to 2014 August menstrual Tianjin xianshuigu hospital diag- nosed by colonoseope to observe the surface morphology of vascular polyp of narrow band imaging of 69 cases with colon polyps(CP), is used to predict the pathological type, the polyp resection specimens for pathologi- cal examination, to study the relationship between surface morphology and pathological blood vessels of the. Results of the 69 patients with 163 lesions, according to the results of pathological diagnosis, a total of 93 tumor lesions, 70 non neoplastic lesions, NBI observation of colon lesions, the sensitivity was 93.50%, specificity was 81.40%, accuracy was 88.34%. The diameter of 〈5mm lesions, the sensitivity was 94.40%, specificity was 96.00%, accuracy was 95.19%; at the same time, the diameter of 〉5mm lesions, the sensi- tivity was 92.30%, specificity was 45.00%, accuracy was colon lesions form of CP to predict the pathological type, neoplastic lesions with a more reliable value. 76.27%.
