早期不同时间窗低分子肝素对脑血栓形成患者血液高凝状态及转归的影响

Effect of low-molecular-weight heparin in early different time windows on hypercoagulable state and prognosis of patients with cerebral thrombosis

目的：研究早期不同时间窗低分子肝素治疗对脑血栓形成血液状态、转归的影响。方法将长治市人民医院收治的120例脑血栓患者抽签随机分为 A、B、C 三组,每组40例,分别在第1、2、3天给予低分子肝素治疗,观察三组患者血小板计数（PLT）、纤维蛋白原（FIB）、血浆凝血酶原时间（PT）、活化部分凝血活酶时间（APTT）改变及临床疗效。结果治疗前后三组 PLT、PT、APTT 均无显著改变（P 〉0.05）,FIB 三组治疗后分别为（1.98±0．24）、（2.46±0.51）、（2.89±0.63）g/ L,均显著低于对照组,A 组显著低于另外两组,组间比较差异均有统计学意义（P 〈0.05）；三组临床效果比较,A、B、C 组分别为92.50%、82.50%、72.50%,其中 A、C 组比较差异有统计学意义（P 〈0.05）。结论低分子肝素可改善患者血液高凝状态,具有较高的安全性,且越早应用对转归越有利。

Objective To observe the effect of low-molecular-weight heparin in early different time windows on blood status and prognosis of patients with cerebral thrombosis. Methods The 120 patients in Changzhi people’s hospital were randomly divided into three groups （ group A, group B and group C）, they were respectively given the treatment of low-molecular-weight heparin on the first, second and third day. And then the changes of platelet count （PLT）, fibrinogen （FIB）, prothrombin time （PT）, activated partial thromboplastin time （APTT） and clinical efficacy were observed. Results Before and after treatment, PLT, PT, APTT of the three groups had no significant change （P 〉0. 05）, FIB after treatment of the three groups respectively was （1. 98 ± 0. 24） g/ L, （2. 46 ± 0. 51） g/ L and（2. 89 ± 0. 63）g/ L, they were all significantly lower than those of the control group, and those of group A was significantly lower than those of the others, the differences between the three groups were significant （P 〈 0. 05）; the clinical efficacy of group A, group B and group C respectively was 92. 50% , 82. 50% and 72. 50% ,and the difference between group A and group C was significant（P 〈 0. 05）. Conclusions Low-molecular-weight heparin can improve patient’s hypercoagulable state, it has high security, and it is better for prognosis if it is gotten a sooner application.
