64排容积 CT 全脑灌注成像对脑血管病变的诊断价值

摘要 目的:探讨64排容积 CT 全脑灌注成像对脑血管病变的诊断价值。方法选取2013年1月至2014年1月接受64排容积 CT 全脑灌注成像的脑血管病变患者81例,回顾分析其检查结果。结果81例患者中32例(39.5%)为动脉瘤,16例(19.8%)为血管畸形,33例(40.7%)为脑血管狭窄或者闭塞,均经 DSA 检查或者手术病理检查证实;脑血管狭窄或者闭塞患侧 rCBF、rCBV 显著低于健侧,而 MTT 显著高于健侧;溶栓治疗后 rCBF、rCBV较治疗前显著提高,MTT 显著降低。结论64排容积 CT 全脑灌注成像用于诊断和随访脑血管病变具有重要意义,可全面评估脑血管病变患者的病情,值得推广应用。 Objective To investigate the diagnostic value of 64 row volume CT imaging whole brain perfusion on cerebral vascular disease. Methods From January 2013 to January 2014, 81 patients with cerebrovascular pathological changes accepted 64 row volume CT cerebral perfusion imaging, the test results were analyzed. Results Among the 81 pa-tients, 32 cases (39. 5% ) of the aneurysm, 16 cases (19. 8% ) were vascular malformation, 33 cases (40. 7% ) of cere-bral vascular stenosis or occlusion, which were confirmed by DSA examination or surgery pathological examination; the in-cidence of cerebrovascular stenosis or occlusion was significantly lower than that of the contralateral side rCBF, rCBV and determined by MTT was significantly higher than the healthy side (P 〈 0. 05); RCBF after thrombolysis treatment, rCBV was significantly increased, and the determined by MTT decreased significantly (P 〈 0. 05). Conclusions 64 row volu-metric CT cerebral perfusion imaging for diagnosis and follow-up of cerebrovascular disease is of great significance, the pa-tient can fully evaluate cerebrovascular pathological changes, so it is worthy of popularization and application.
