螺旋CT鉴别诊断厚壁型胆囊癌与慢性胆囊炎

Differential Diagnosis of Thick Wall Gallbladder Carcinoma and Chronic Cholecystitis by Spiral CT

目的：对厚壁型胆囊癌以及慢性胆囊炎的螺旋CT诊断结果进行对比分析，为临床中两种疾病的诊断提供可靠的参考依据。方法根据我院2010年1月～2014年6月接收的厚壁胆囊癌患者32例和慢性胆囊炎患者66例来进行研究分析，对患者的临床CT诊断图像进行分析，比较诊断准确率。结果厚壁胆囊癌和慢性胆囊炎患者的临床胆囊厚度存在统计学差异性，前者的厚度比后者要大。慢性胆囊炎患者的粘膜完整率、轮廓清晰度要比厚壁胆囊癌患者要大，差异具有统计学意义， P＜0.05，两种疾病患者的胆道阻塞几率和位置有差异性，厚壁胆囊癌患者的发生几率比慢性胆囊炎患者要高。结论使用螺旋CT进行厚壁型胆囊癌和慢性胆囊炎的扫描诊断，可以观察到两者的差异性，临床中能够使用该种诊断方式来进行区分鉴别。

Objective To analyze the diagnostic results of CT in thick wall gallbladder carcinoma and chronic cholecystitis, and provide reliable reference for the diagnosis of two kinds of diseases. Methods 32 patients with thick wall gallbladder carcinoma received in our hospital from January 2010 to June 2014, 66 patients with chronic cholecystitis and cases of chronic cholecystitis were analyzed, and the clinical CT diagnostic images were analyzed, and the diagnostic accuracy was compared. Results There was statistical difference in clinical gallbladder thickness between thick wall gallbladder carcinoma and chronic cholecystitis, the former was more than the latter. The mucosal integrity rate of patients with chronic cholecystitis, contour clarity than patients with thick wall gallbladder carcinoma, the results were statistically signiifcant, P〈0.05, two kinds of disease patients with different risk and location of bile duct obstruction, the incidence of thick wall gallbladder cancer patients with chronic cholecystitis. Conclusion Using spiral CT for the diagnosis of thick wall gallbladder carcinoma and chronic cholecystitis, the difference between the two can be observed, and the clinical diagnosis can be used to distinguish between the two.
