雷公藤多苷片治疗糖尿病肾病大量蛋白尿的疗效观察

Effect of A Large Amount of Proteinuria in the Treatment of Triperygium Wilfordii Multiglucoside

目的：探讨雷公藤多苷片治疗糖尿病肾病大量蛋白尿的临床疗效。方法选取2013年10月～2014年11月我院收治的糖尿病肾病患者58例，随机分为观察组29例，对照组29例，对照组患者给予常规糖尿病肾病治疗，观察组患者在常规治疗基础给予雷公藤多苷片治疗，比较两组患者治疗前、后血清白蛋白、血肌酐、24 h尿蛋白定量、血尿素氮、免疫学指标及临床症状积分变化情况。结果治疗后，观察组患者血清白蛋白水平高于对照组，而24 h尿蛋白定量水平低于对照组，差异有统计学意义（P＜0.05）；治疗后两组患者血肌酐及血尿素氮水平比较，差异无统计学意义（P＞0.05）；治疗后观察组患者血清C3、C4、IgA及IgG免疫学指标与对照组比较，明显改善，差异有统计学意义（P＜0.05）；治疗后两组患者临床症状积分均降低，且观察组患者症状积分低于对照组，差异有统计学意义（P＜0.05）。结论雷公藤多苷片治疗糖尿病肾病，能够有效提高血清白蛋白水平，减少尿蛋白水平，改善血清C3、C4、IgA及IgG免疫学指标水平，降低临床症状积分，有利于提高糖尿病肾病患者疾病治疗效果。

Objective To investigate tripterygium glycosides tablet treat diabetic nephropathy proteinuria clinical efficacy. Methods 58 cases diabetic patients were chosen from October 2013 to November 2014 in our hospital, 29 cases were randomly divided into two groups, a control group of 29 patients, control group were given routine treatment of diabetic nephropathy, the observation group were given conventional treatment tripterygium glycosides tablet treatment, comparison of two groups of patients before and after treatment serum albumin, serum creatinine, 24 hour urinary protein excretion, blood urea nitrogen, immunological parameters and clinical symptom score changes. Results After treatment, the observation group was higher serum albumin levels, while the 24 hour urinary protein levels lower than the control group, the difference was statistically signiifcant （P〈0.05）. After treatment, patients were compared serum creatinine and blood urea nitrogen levels, the difference was not statistically signiifcant （P〉0.05）. After treatment, patients observed in serum C3, C4, IgA and IgG immune indexes in comparison with the control group, improved signiifcantly, the difference was statistically signiifcant （P〈0.05）. After treatment, clinical symptoms of both groups were lower points, and the symptoms observed in patients group points lower than in the control group, the difference was statistically significant （P〈0.05）. Conclusion Tripterygium glycosides tablets in the treatment of diabetic nephropathy, increased serum albumin level can effectively reduce urinary protein levels, improved serum C3, C4, IgA and IgG levels immunological parameters, reduce the clinical symptom score, help to improve the treatment of diabetes kidney disease patients.
