远端万向锁定加压双柱接骨板治疗桡骨远端骨折 被引量:13

摘要 目的 探讨远端万向锁定加压双柱接骨板治疗桡骨远端骨折的疗效.方法 回顾性分析2009年12月至2014年12月采用远端万向锁定加压双柱接骨板治疗的58例桡骨远端骨折患者资料,男17例,女41例;年龄52 ~ 67岁,平均58.7岁;骨折按AO/OTA分型:C1型20例,C2型18例,C3型20例.末次随访时通过术后X线片评估桡骨远端骨折复位情况、测量各项影像学参数,并采用Gartland-Werley功能评分系统评定疗效. 结果 所有患者术后获4~57个月(平均30.6个月)随访.X线片示骨折愈合时间为2~3个月,平均2.5个月.末次随访时X线片示关节面平整度均<2 mm,掌倾角平均为13.63°(11.00°~ 15.95°)、尺偏角21.01°(18.20°~24.36°)、桡骨茎突高度11.53 mm(9.60 ~ 13.82mm)、背伸67.60°(62.59° ~72.61°)、掌屈57.75°(51.48°~62.02°)、尺偏29.75°(25.69°~ 32.81°)、桡偏20.96°(18.95°~ 22.97°)、旋前78.01°(73.8°~81.22°)、旋后72.65°(66.75°~ 75.98°),恢复满意;采用Gartland-Werley功能评分评定疗效:优26例,良26例,可6例,优良率为89.7%.无伤口感染患者或桡动脉受损伤患者,其中有1例患者术后出现正中神经损伤表现. 结论 采用远端万向锁定加压双柱接骨板治疗桡骨远端骨折可获得满意的临床疗效.
