摘要
目的通过Meta分析系统评价肿瘤组织中脱嘌呤脱嘧啶核酸内切酶1(APE1)的免疫组化表达,进一步探讨其与恶性肿瘤的关系及临床意义。方法检索PubMed、EMBASE、中国期刊全文数据库、万方数据库,检索时间为建库至2015年2月,使用stata13.0软件对符合纳入标准的研究结果进行Meta分析。结果共纳入36篇文献,包括3 374例患者和521例正常对照。Meta分析显示,与正常组织相比,肿瘤组织中APE1出现胞浆异位表达(OR=17.82);而且,APE1表达不论是阳性细胞数量,还是表达水平均显著高于正常对照组(OR=37.56,OR=47.56)。APE1的异位表达主要在卵巢上皮癌、胰腺癌、大肠癌、膀胱移行细胞癌和肺癌;APE1阳性表达增高主要在肝癌、胰腺癌、膀胱移行细胞癌、外周T细胞淋巴瘤和非小细胞肺癌。APE1在淋巴结转移肿瘤组的阳性表达高于未转移组(OR=4.10);APE1高表达的肿瘤患者,其化疗敏感性较低表达患者差(OR=6.45)。结论 APE1的异位表达和(或)表达量增加与多种恶性肿瘤的发生、发展及预后具有显著的相关性。APE1可能是肿瘤诊断及预后的潜在标志物。
Objective To analyze and evaluate the immunohistochemical expression of apurinic/aprimidinic endonuclease 1(APE1)and to further investigate its association with malignant tumors through meta-analysis.Methods According to selection criteria,all articles were systematically retrieved from the PubMed,EMBASE,CNKI and Wanfang data databases from their establishment to Feb.2015.The research results conformed to the included standards were performed the statistical analysis by the Stata13.0software.Results A total of 36 eligible literatures involving 3 374 patients and 521 normal controls were included.The meta analysis revealed that APE1 showed a translocation expression in cancer tissue compared with the normal tissue(OR=17.82),furthermore,both the positive cells number and the expression level of APE1 in cancer tissue were significantly higher than those in the control group(OR=37.56,OR=47.56).The ectopic expression of APE1 was predominantly found in epithelial ovarian cancer,pancreatic carcinoma,colorectal tumor,bladder transitional cell carcinoma and lung cancer;the positive expression increase of APE1 was mainly in hepatocellular carcinoma,pancreatic cancer,bladder transitional cell carcinoma,peripheral T cell lymphoma and non-small cell lung cancer.The analysis suggested that the expression in the lymph nodes metastasis tumor group was higher than that in the non non-lymph nodes metastasis tumor group(OR=4.10).Besides,the chemotherapeutical sensitivity in the patients with high APE1 expression was poorer than that in the patients with low APE1expression(OR=6.45).Conclusion The APE1 ectopic expression and/or expression increase have significant correlation with the occurrence,progression and prognosis of multiple tumors.APE1 may be a potential marker for the diagnosis and prognosis of tumors.
