摘要
目的探讨脑红蛋白(neuroglobin,NGB)及N端脑钠肽前体(N-terminal pro-brain natriuretic peptide,NT-pro BNP)在心肺脑复苏中的变化与意义。方法 126例心搏骤停(cardiac arrest,CA)患者按抢救结果分为A组:心肺脑复苏30 min无自主循环,或有自主循环但<48 h;B组:自主循环恢复≥48 h,未能存活出院;C组:抢救成功,存活出院。于心肺脑复苏0 h(即刻)、3 h、12 h、24 h、48 h留取静脉血,测定血清中NGB和血浆中NT-pro BNP的含量。结果在0 h,NT-pro BNP无明显变化,各组比较,P>0.05,而NGB,各组比较,P<0.05;B组NGB、NT-pro BNP表达随时间延长总趋势明显增高,而C组则是明显降低,C组与B组比较,P<0.05,其中NGB在B组中的12 h有降低,NT-pro BNP在C组中的12 h有增高。结论检测血液中NGB、NT-pro BNP可反映心肺脑复苏的效果与预后,但NGB的表达更提前。
Objective To investigate the change and significance of neuroglobin (NGB) and N-terminal pro-brain natriuretic peptide (NT-proBNP) during cardiopulmonary cerebral resuscitation (CPCR). Methods 126 patients with cardiac arrest (CA) were selected in this study. According to saving results, all patients were divided into three groups: (1)Group A: without recovery of spontaneous circulation (ROSC) after 30 min of CPCR, or ROSC 〈 48 h; (2)Group B: ROSC ≥ 48 h, but failed to survive ; (3)Group C: successful rescue, live out of the hospital. The levels of NGB and NT-proBNP in blood were determined at the time of 0, 3, 12, 24, 48 h after CPCR. Results The level of NT-proBNP had no significant difference of each group at 0 h (P 〉0.05), while the level of NGB had obvious significant difference at 0 h among each group (P 〈0.05). The general expression trends of NGB and NT- proBNP in group B were significantly increased with the extension of time, and decreased significantly in group C; The differences between group B and group C were statistically significant (P〈0.05); In which, the level of NGB decreased in group B at 12 h, and NT-proBNP increased in group C at 12 h. Conclusions Detection of NGB and NT-proBNP in the blood can reflect the effect and prognosis of CPCR, but the expression of NGB is more in advance.
