摘要
目的探讨尿微量蛋白(MALB)、α1-微球蛋白(α1-MG)和β2-微球蛋白(β2-MG)、转铁蛋白(TRF)、尿视黄醇结合蛋白(RBP)联合检测在糖尿病肾病诊断中的应用价值。方法随机选取2013年1月至2013年6月我院接收的糖尿病肾病患者100例作为观察组,选取同时期的健康体检者100例为对照组,采用免疫比浊法分别测定其MALB、α1-MG、β2-MG、TRF、RBP水平,观察组同时测定MALB、α1-MG、β2-MG、TRF、RBP水平以及Cys-C、血Cr、尿蛋白定性的阳性率。结果观察组MALB、α1-MG、β2-MG、TRF、RBP水平明显高于对照组,观察组MALB、α1-MG、β2-MG、TRF、RBP阳性率明显高于Cys-C、血Cr、尿蛋白定性的阳性率,且α1-MG、β2-MG、RBP联合检测的阳性率明显高于MALB、α1-MG、β2-MG、TRF单一指标的阳性率,其差异具有统计学意义(P<0.05)。结论 MALB、α1-MG、β2-MG、TRF、RBP五项联合检测有助于糖尿病肾病患者的早期诊断,临床应加以推广。
Objective To investigate the application value of combined detection of microalbuminuria (MALB), α1-microglobulin (α1-MG) and β2-microglobulin (β2-MG), transferrin (TRF), urinary retinol binding protein (RBP) in the diagnosis of diabetic nephropathy (DN). Methods 100 DN patients admitted to our hospital from January 2013 to June 2013 were selected as the observation group, and 100 cases of healthy persons at the same period were selected as the control group. The levels of MALB, α1-MG, β2-MG, TRF and RBP of both groups were detected by immunonephelometry, and the positive rates of MALB, α1-MG,β2--MG, TRF, RBP, Cys-C, blood Cr and urinary protein qualitative detection of observation group were observed. Results The levels of MALB,α1-MG, β2--MG, TRF and RBP of observation group were significantly higher than those of control group (P 〈0.05). In the observation group, the positive rates of MAL, α1-MG, β2--MG, TRF and RBP were significantly higher than the positive rates of Cys-C blood Cr and urinary protein qualitative detection, and the positive rate of combined detection of α1-MG, β2--MG and RBP was significantly higher than the positive rates of MALB, α1-MG, β2-MG, TRF; All the differences were statistically significant (P 〈0.05). Conclusions The combined detection of MALB, α1-MG,β2-MG, TRF and RBP can help early diagnosis of diabetic nephropathy.
