单纯修补术与胃大部分切除术治疗急性胃穿孔的临床比较

Clinical Comparison Between Simple Repair Surgery and Subtotal Gastrectomya Surgery for the Treatment of Acute Gastric Perforation

目的比较单纯修补术与胃大部分切除术治疗急性胃穿孔的临床疗效。方法选取我院收治的急性胃穿孔患者120例,将其随机分为A组和B组,每组各60例,A组采用单纯修补术,B组采用胃大部分切除术。比较两组患者的手术时间、胃肠功能恢复正常时间、下床活动时间、术中出血量、住院时间及术后并发症的发生率。结果 A组治疗后手术时间、胃肠功能恢复正常时间、下床活动时间及住院时间均明显短于B组(P<0.01),A组术中出血量也明显少于B组(P<0.01)。A组术后并发症的发生率为6.7%,B组为23.3%,两组比较差异具有统计学意义(P<0.05)。结论单纯修补术相比于胃大部分切除术疗效更加显著,术后患者恢复快,创伤较小,术后并发症少。

Objective To compare the clinical effects of simple repair surgery and subtotal gastrectomya surgery for the treatment of acute gastric perforation. Methods 120 cases of patients with acute gastric perforation admitted to our hospital were randomly divided into group A and group B, with 60 cases in each group. Group A was given simple repair surgery, while group B was given subtotal gastrectomy surgery. The differences in the operative time, time to return to normal gastrointestinal function, ambulation time, intraoperative blood loss, length of hospital stay and incidence of postoperative complications between two groups were analyzed. Results The operative time, time to return to normal gastrointestinal function, ambulation time and length of hospital stay of group A were significantly shorter than those of group B （P〈0.01）, and the intraoperative blood loss of group A was significantly less than that of group B （P 〈0.01）. The incidence of postoperative complications of group A was 6.7%, significantly lower than 23.3% of group B （P 〈0.05）. Conclusions Compared with subtotal gastrectomya surgery, simple repair surgery has more significant efficacy, with rapid postoperative recovery, less trauma and fewer postoperative complications.
