关节镜下经皮微创张力带内固定治疗髌骨骨折

Patella fracture treated by skin tension belt under arthroscopes

目的探讨膝关节镜辅助下经皮穿刺克氏针张力带固定技术治疗单纯髌骨骨折的临床疗效。方法对2011年1月～2014年2月收治的单纯髌骨骨折69例,随机分为两组,处理微创组38例,男性24例,女性14例;年龄19～65岁,平均（31.4±0.3）岁。对照组31例,男性17例,女性14例;年龄18～61岁,平均（30.3±0.2）岁。处理微创组采用关节镜辅助下经皮穿刺克氏针张力带内固定术治疗,对照组为传统手术开放复位,比较两组住院时间、手术时间、愈合时间、关节功能评分。结果经半年以上随访,处理微创组平均住院时间和骨折愈合时间均低于对照组（P〈0.01）,处理微创组膝关节功能优于对照组（P〈0.05）。结论膝关节镜辅助下经皮穿刺克氏针张力带固定术治疗单纯髌骨骨折固定牢固,创伤小,恢复快,能同时探查和处理膝关节其他损伤,值得临床推广。

Objective To observe the effect of simple patella fracture patients treated by skin tension belt under arthroscopes.Methods Totally 69 cases of simple patella fracture from Jan.2011 to Feb.2014 were separa-ted into 2 groups.Thirty-eight cases in the experiment group were treated by skin tension belt under arthroscopes and 31 cases in the control group received routine open operation.In the experiment group,24 were male and 13 were female with an average age of （31.4 ±0.3）years （19-65 year）.In the control group,17 were male and 14 were female with an average age of （30.3 ±0.2）years （18-61 year）.The hospitalization time,operation time,heal-ing time and joint function score were compared between the two groups.Results After follow-ups of more than half a year,the hospital stay and healing time in the experiment group were significantly less than those in the control group（ P〈0.01） .The knee function in the experiment group was significantly better than that in the control group （ P〈0.05） .Conclusion Skin tension belt under arthroscopes for the treatment of simple patella fracture patients is worthy of wide clinical promotion for having advantages as less invasion,stable fixation,quick function recovery as well as exploration and management of combined injuries at the same time.
