帕金森病的数字化四诊属性特征研究

Study on the digital four diagnostic characteristics in patients with Parkinson's disease

目的:探讨帕金森病(PD)患者的数字化四诊属性特征,探索建立四诊合参辅助诊疗仪对PD及中医颤证的数字化量化辨识系统。方法:应用BD-SZ型四诊合参辅助诊疗仪,采集36例PD患者的舌诊、脉诊、闻声诊及问诊信息,提取和识别其数字化特征信息,并进行中医辨证。结果:36例受试者中,舌象主要为淡红舌17例,薄苔21例,白苔27例,干燥苔13例,水滑苔11例;脉象主要为脉沉22例,脉弦21例;闻声诊辨识五行体质,属木型体质者11例,土型9例,水型8例,火型7例,金型1例;出现频率较高的主症有动作迟缓32例、肢体震颤30例,兼症有疲劳乏力26例,腰膝酸软24例;主要辨证分型为肝风内动15例,气血亏虚和肝肾不足各7例。结论:PD患者的数字化四诊特征主要表现为淡红舌薄白苔,脉图多呈沉弦状态,五行体质多属木型。

Objective： To study the digital four diagnostic characteristics in patients with Parkinson＇s disease （PD）, and to establish a quantitative identification system of PD and tremor syndrome in TCM Four Diagnosis Auxiliary Apparatus. Methods： 36 patients with PD were selected for the study. The information of their tongue, pulse, sound and inquiring was collected by the BD-SZ TCM Four Diagnosis Auxiliary Apparatus. Its digital characteristic and TCM syndromes were extracted and identified. Results： There were 17 cases with slight red tongue, 21 cases with thin coating, 27 cases with white coating, 13 cases with dry coating, 11 cases with watery coating. Deep pulse was identified in 22 cases and wiry pulse in 21 cases. 11 cases were identified as wood type constitution, 9 as earth type, 8 as water type, 7 as fire type and 1 as metal type. Their primary symptoms were slow movement （32 cases）, tremor of limbs （30 cases）, fatigue （26 cases）, and weakness of lower back and knees （24 cases）. The main syndromes of TCM were up-stirring of liver wind type （15 cases）, qi and blood deficiency （7 cases）, and liver and kidney deficiency （7 cases）. Conclusion： The digital four diagnostic characteristics of PD patients mainly manifested as slight red tongue with thin white coating, deep and wiry pulse, and wood type constitution.
