摘要 【目的】探讨血清肺表面活性物质相关蛋白 D(SP-D)、人克拉拉细胞蛋白(CC16)在慢性阻塞性肺疾病急性加重(AECOPD)患者中表达水平及其意义。【方法】选择 AECOPD 患者69例(观察组),另外选取同期健康体检者50例(对照组),采用双抗体夹心法(ELISA)检测两组治疗前后血清 SP-D、CC16水平和肺功能指标如用力肺活量(FVC)、第1秒用力呼气容积(FEV1)、FEV1/FVC 及 FEV1占预计值的百分比(FEV1%),并分析 SP-D、CC16水平与肺功能指标的相关性。【结果】与对照组相比,AECOPD 患者的血清 SP-D 水平显著升高,而 CC16水平显著下降;经治疗病情好转后,患者血清 SP-D 水平下降,而 CC16水平上升,但仍未到达对照组的正常水平;治疗后,观察组 FEV1%及 FEV1/FVC 均升高,但仍低于对照组,差异有统计学意义(P <0.05)。SP-D 与 FEV1%及 FEV1/FVC 呈负相关性(P <0.05);CC16与 FEV1%及 FEV1/FVC 呈正相关性(P <0.05)。【结论】SP-D、CC16的水平是反映 AECOPD 疗效的重要指标,可通过检测血清 SP-D、CC16水平可评估 COPD 患者病情严重程度。 [Objective]To investigate clinical significance and the changes of levels of surfactant protein D (SP-D)and clara cell protein(CC1 6)in patients with acute exacerbation of chronic obstructive pulmonary disease (AE-COPD).[Methods]From April 2013 to April 2014,69 AECOPD patients who were achieved after treatment in our hospital were selected as the case group,while 50 cases of healthy people at the same period in our hospital were selected as the control group.The plasma levels of SP-D and CC1 6 in two groups before and after treatment were measured by enzyme linked immunosorbent assay (ELISA).The changes of lung function index were com-pared.Pulmonary function index such as FVC,forced expiratory volume (FEV1 ),FEV1/FVC and estimated FEV1 percentage (FEV1 %),and the correlation of serum SP-D and CC1 6 levels with lung function index were an-alyzed.[Results]Compared with the control group,the serum SP-D level was significantly elevated in patients with AECOPD,while the level of CC1 6 decreased significantly.After therapy,conditions of patients were getting bet-ter,the serum level of SP-D decreased,and the level of CC1 6 increased,but still did not reach the regular levels of the control group.The levels of FEV1/FVC and FEV1 % in the case group were significantly higher after treat-ment ,but still lower than those in the control group,the difference was statistically significant (P 〈0.05).SP-D was negatively correlated with FEV1 % and FEV1/FVC (P 〈0.05);CC1 6 was positively correlated with FEV1 %and FEV1/FVC (P 〈0.05).[Conclusion]The levels of CC1 6 and SP-D were important index to reflect the effica-cy of AECOPD,and can be used to evaluate the severity of COPD and has important clinical significance for the e-valuation of AECOPD patients.
