可吸收止血结扎夹在脾脏切除术中的临床应用价值

Clinical Application Value of Absorbable Hemostatic Clips in Patients Undergoing Splenectomy

【目的】探讨脾脏切除术中应用可吸收止血结扎夹的临床疗效及其应用价值。【方法】选择2013年1月至2014年7月本院收治的接受脾脏切除术的52例患者，根据止血方法的差异分为观察组（22例），术中应用人工合成可吸收止血结扎夹；对照组（30例），术中应用常规丝线打结止血。比较两组的手术时间、住院时间、术中输血量、术后引流量的差异。随访1个月，比较两组患者的二次手术发生率及腹腔积液、腹腔感染、出血等并发症的发生率差异。【结果】与对照组比较，观察组的手术时间、住院时间、术中输血量及术后引流量均显著较少（均 P ＜0.05）。随访1个月，观察组患者的二次手术发生率及各种并发症的发生率均显著低于对照组（均 P ＜0.05）。【结论】在脾脏切除术中应用可吸收止血结扎夹具有较好的安全性，缩短了手术及住院时间，减少了术后并发症，具有较好的临床疗效，是一种值得推广的止血方法。

[Objective]To explore the clinical value of the absorbable hemostatic clamps in splenectomy.[Methods]A total of 52 patients undergoing splenectomy in our hospital from Jan.2013 to July 2014 were cho-sen.According to the difference of hemostatic methods,all patients were divided into observation group（n =22）receiving synthetic absorbable hemostatic clips in operation and control group（n =30）receiving the rou-tine knotted hemostasis with silk in operation.The operation time,hospital stay,intraoperative blood transfu-sion amount and postoperative drainage were compared between two groups.During 1 month of follow-up,the occurrence of reoperation and the complications such as seroperitoneum,abdominal infection and hemorrhage were compared between two groups.[Results]Compared to control group,the operation time,hospital stay, intraoperative blood transfusion amount and postoperative drainage in experimental group were lower signifi-cantly（P〈0.05）.During 1 month of follow-up,the occurrence of reoperation and the complications such as se-roperitoneum,abdominal infection and hemorrhage in experimental group were significantly lower than those in control group（P 〈0.05）.[Conclusion]The absorbable hemostatic clips in splenectomy is very safe,which can shorten the time of surgery and hospitalization and reduce the postoperative complications.It has good clin-ical efficacy,so it is a kind of method for hemostasis and worth promoting.
