自制排石汤联合体外冲击波碎石治疗输尿管壁段结石临床研究

THE CLINICAL RESEARCH OF THE SELF-MADE PAI SHI DECOCTION COMBINATION WITH ESWL IN TREATMENT OF PATIENTS WITH URETEROVESICAL JUNCTION STONES

目的探讨自制排石汤治疗输尿管壁段结石的临床疗效。方法选海慈医院输尿管壁段结石患者290例,均已经过影像学诊断明确,根据患者意愿分为A、B、C三组:A组口服排石汤治疗、B组体外冲击波治疗组、C组体外冲击波+排石汤治疗组。随访患者两周后对各组排石率、排石时间、并发症发生率进行统计学分析(P<0.05有统计学意义)。结果 A组排石率56.3%,排石时间8.8±1.6天,并发症发生率71.3%。B组排石率77.6%,排石时间(6.9±1.2)d,并发症发生率96.4%。C组排石率91.8%,排石时间(4.7±1.3)d,并发症发生率43.8%。C组的排石率(91.8%)明显提高,而且排石时间(4.7±1.3)d、并发症发生率(43.8%)等均优于其他两组,差异有统计学意义(P<0.05)。结论自制排石汤联合体外冲击波碎石能显著提高输尿管壁段结石的排石率,缩短排石时间,减少并发症的发生。

Objective To evaluate the effects of the Self-made Pai Shi decoction in the treatment of uret-erovesical junction stones. Methods All 290 patiens from Hiser hospital, Confirming by imaging examina- tion, were divided into three groups by patients willing- ness ：patients received Self-made Pai Shi decoction（Group A,n= 98）;patients received ESWL（Group B,n=110） patients received Self-made Pai Shi decoc- tion combination with ESWL（Group C,n=82）. We recorded and evaluated the stone-clearance rate,the stone-clearance time and the complication rates after 2 weeks follow-up. Results The stone-clearance rate in group A is 56. 3%,the stone-clearance time is （8. 8±1. 6） days,the complication rate is 71. 3%. The stone-clearance rate in group B is 77.6% ,the stone-clearance time is （6. 9±1.2） days, the complication rate is 76.4%. The stone-clearance rate in group C is 91.8% ,the stone-clearance time is （4.7±1.3） days, the complication rate is 43.8%. The stone-clearance rate in group C is significantly greater compared to theA,B group,in addition, the stone-clearance time and the complication rates are significantly lower in group C. the difference have statistics significance（ P 〈0.05）. Conclusion The Self-made Pai Shi decoction combination with ESWL is effective in removing ureterovesical junction stones, It shortens the stone-clear- ance time, decreases the complication rates and slso improves stone-clearance rate.
