重型颅脑损伤患者去骨瓣减压术后并发对侧硬膜下积液的处理 被引量：14

摘要 目的总结重型颅脑损伤(TBI)患者去骨瓣减压术(DC)术后并发对侧硬膜下积液(SDG)的治疗经验。方法选择19例DC术后并发对侧SDG的TBI患者,均采取保守治疗,9例积液占位效应明显者同时行手术治疗。手术方式钻孔引流2例(1例引流失败改行骨瓣开颅积液清除术),骨瓣开颅积液清除术1例,硬膜下腹腔分流术4例,硬膜下腹腔分流术加颅骨修补2例。结果 19例患者SDG均消失,无术后并发症发生;随访6个月,Glasgow预后评分2分2例、3分8例、4分9例。结论 TBI患者DC术后并发对侧SDG,积液占位效应不明显者可先采取保守治疗,占位效应明显者需行手术治疗。
