多层螺旋CT检查诊断宫颈癌淋巴结转移的临床研究 被引量:20

摘要 目的:探讨多层螺旋CT检查诊断宫颈癌淋巴结转移临床价值,以期选择最佳方法,提高临床诊治水平。方法:选取2010年1月-2013年1月46例宫颈癌淋巴结转移患者,对宫颈癌髂总、髂内和髂外、腹总动脉旁淋巴结转移进行诊断,将多层螺旋CT检查数目和淋巴结清扫术病理检查结果进行分析。结果:对多层螺旋CT诊断淋巴结转移敏感性和特异性进行比较,髂总动脉敏感性为88.75%,特异性为68.93%;髂内动脉敏感性为87.67%,特异性为67.56%;髂外动脉敏感性为90.77%,特异性为73.15%;腹主动脉旁敏感性为92.14%,特异性为72.43%。多层螺旋CT诊断淋巴结转移总敏感性为89.83%,总特异性为70.52%。对多层螺旋CT和病理诊断淋巴结转移比较,CT诊断阴性符合率为63.04%,阳性符合率为36.96%;病理阴性符合率为30.43%,阳性符合率为69.57%,两者比较差异无统计学意义(P>0.05)。结论:多层螺旋CT可作为诊断宫颈癌淋巴结转移的首选诊断方法,其诊断价值较高。 Objective:To investigate the clinical value of multislice spiral CT in the diagnosis of cervical lymph node metastasis.Methods:All 46 cases of cervical lymph node metastasis in patients with cervical cancer iliac,inter-nal iliac and external iliac,abdominal artery bypass diagnosis of lymph node metastasis,the number of multislice spiral CT examination and lymph nodes dissection pathology results were analyzed.Results:The sensitivity and specificity of multislice CT diagnosis of lymph node metastasis were compared.Sensitivity was 88.75% of the common iliac artery and specificity was 68.93%.Sensitivity was 87.67% of the internal iliac artery and specificity was 67.56%.The ex-ternal iliac artery sensitivity was 90.77% and a specificity 73.15%.Sensitivity was 92.14% of abdominal aortic ar-tery and specificity was 72.43%.Multislice spiral CT diagnosis of lymph node metastasis overall sensitivity was 89.83%,specificity 70.52%.Multislice spiral CT for the diagnosis of lymph node metastasis and pathological com-parison,CT diagnosis of negative coincidence rate was 63.04%,the positive rate was 36.96%.Pathology negative co-incidence rate was 30.43%,the positive rate was 69.57%(P >0.05).Conclusion:Multislice spiral CT can be used as a diagnostic method for cervical lymph node metastasis,with high diagnostic value.
