中性粒细胞与淋巴细胞比值与膀胱尿路上皮癌预后的相关性 被引量:4

摘要 目的探讨血中性粒细胞与淋巴细胞比值(NLR)与膀胱尿路上皮癌预后的关系。方法选择2006年9月—2009年9月泌尿外科住院的膀胱尿路上皮癌100例,术前常规检测NLR,分为低NLR组(NLR≤2.7分)和高NLR组(NLR>2.7分)。门诊或电话随访记录两组存活和肿瘤复发情况,比较两组临床病理参数并进行相关性分析。结果本研究显示低NLR组1、3、5年生存率明显高于高NLR组,肿瘤复发率均低于高NLR组,组间比较差异均有统计学意义(P<0.05)。单因素分析显示年龄、肿瘤浸润深度、淋巴结转移、分化差是影响预后的高风险因素,多因素分析显示高NLR是影响术后肿瘤患者预后的独立危险因素。结论高NLR为影响膀胱尿路上皮癌患者预后的危险因素,NLR值越高预后越差,NLR对判断膀胱尿路上皮癌的预后具有较高的应用价值。 Objective To investigate the relationship between blood neutrophils and lymphocytes ratio (NLR) and the prognosis of bladder urothelial carcinoma. Methods 100 cases of bladder urothelial carcinoma of our hospital during sep-tember 2007 and September 2011 underwent preoperative routine blood test NLR. According to the ratio of NLR, the patients were divided into low NLR group (NLR≤2. 7) and high NLR group (NL〉2. 7). Clinical pathological parameters, of the two groups were compared and telephone follow-up, combined with data of tumor recurrence and survival were analyzed in terms of correlation. Results This study revealed that 1, 3, 5-year survival rate and tumor recurrence rate of low NLR group were significantly higher than that of high NLR group, with statistically significant the differences (P〈0. 05). Single factor a-nalysis showed that age, tumor infiltration depth, lymph node metastasis and poor differentiation were high risk factors affect-ing prognosis and multi-factor analysis showed that high NLR was an independent risk factor affecting the prognosis of patients with postoperative tumor. Conclusion High NLR is a risk factors influencing the prognosis of bladder urothelial carcinoma, and higher the NLR is, the poorer prognosis is. NLR is of great practical value in the prognosis of bladder urothelial carcinoma.
