摘要 目的:综合评价 FcγRIIA 受体131位点多态性与肺炎易感性的关系。方法以“FcgammaRIIa”、“FCGR2A”、“Fcgamma receptor IIA”、“CD32”、“pneumonia”、“polymorphism”、“variant”、“genotype”、“allotype”、“肺炎”、“多态性”为检索词检索PubMed数据库、Embase数据库和中国学术期刊全文数据库,根据标准纳入相关文献,检验各组Hardy‐Weinberg遗传平衡定律,评估各项研究结果的异质性及发表偏倚。结果本研究共纳入6篇病例对照研究,荟萃分析结果表明,FcγRIIA受体131位点基因型(H/H、R/R和 H/R)数据合并后 OR值分别为1.67(95% CI 1.11~2.51)、1.09(95% CI 0.85~1.40)和0.81(95% CI 0.60~1.19),提示FcγRIIA H/H131基因型与肺炎易感性呈正相关,而 R/R基因型和 H/R基因型无相关性。数据合并过程中存在较大异质性( I2分别为75.4%、51.8%和71.9%)。敏感性分析结果表明,本研究结果的稳定性较好(OR值分别为1.34~1.83、1.00~1.22和0.69~0.94)。Egger检验与Begg检验结果提示,H/H基因型和 H/R基因型的研究无发表偏倚,R/R基因型研究可能具有一定偏倚。结论 FcγRIIA H/H131基因型与肺炎易感性呈正相关,而R/R基因型和H/R基因型与肺炎易感性无相关性。 Objective To investigate the association between FcγRIIA H/H131 genotype and pneumonia through a comprehensive meta‐analysis .Methods Databases including PubMed ,Embase ,and China National Knowledge Infrastructure (CNKI) were searched with such terms as “FcgammaRIIa” ,“FCGR2A” ,“Fcgamma receptor IIA” ,“CD32” and“pneumonia” ,“polymorphism” ,“variant” ,“genotype” ,“allotype” ,as well as“pneumonia”and“polymorphism”in Chinese to identify relevant studies .Two reviewers independently extracted the data .Crude odds ratios (ORs) and corresponding 95%confidence intervals (CIs) were estimated .Heterogeneity across studies and publication bias were evaluated and sensitivity analyses were performed .Results Six case‐control studies were finally included in this analysis .The results indicated an association between FcγRIIA H/H131 genotype and pneumonia susceptibility (OR 1 .67 , 95% CI 1 .11‐2 .51 ) , but no significant association with R/R genotype(OR 1 .09 ,95% CI 0 .85‐1 .40) or H/R genotype(OR 0 .81 ,95% CI 0 .60‐1 .19) . Statistically significant heterogeneity was found during data pooling (I2 was 75 .4 % ,51 .8% and 71 .9% ) .Sensitivity analyses further indicated the reliability of the study .Egger′s and Begg′s tests identified publication bias in R/R genotype ,but not in H/H or H/R genotype .Conclusions We preliminarily find an association between FcγRIIA H/H131 genotype and pneumonia susceptibility .
