超声心动图测量正常胎儿心输出量的研究及临床意义 被引量:4

摘要 目的利用胎儿超声心动图测量正常中晚孕期胎儿的心输出量,探讨其变化规律,建立正常中晚孕期胎儿心输出量正常参考值范围,评估胎儿心功能。方法选取2014年11月至2015年5月385例24~40周正常单胎胎儿,测量胎儿大小,估算胎儿体重(EFW),分别测量其主动脉和肺动脉的内径、速度时间积分(VTI),及胎儿心率(HR)、EFW,利用公式分别计算出胎儿左心输出量(LCO)、右心输出量(RCO)、联合心输出量(CCO),计算联合心输出量指数(CCOi)(CCO/EFW)。结果胎儿CCO随孕周的增加而增大,CCOi与孕周无明显相关性,CCOi正常值范围为468~528 ml·min-1·kg-1。结论通过胎儿超声心动图测量胎儿CCO、CCOi方法可行,具有无创性和良好可重复性,胎儿期CCOi有其正常值范围,可作为评价心功能的量化指标。
