胃腺癌组织己糖激酶-Ⅱ的表达及临床意义

Expression and clinical significance of HK-Ⅱ in gastric adenocarcinoma

摘要 目的分析胃腺癌组织己糖激酶-Ⅱ(HK-Ⅱ)的表达水平与化疗相关蛋白(胸苷酸合成酶,TS)、细胞增殖、凋亡指数、患者临床病理特征及预后的关系。方法收集62例胃腺癌组织临床标本,采用免疫组织化学法检测胃腺癌HK-Ⅱ和TS的表达,分析HK-Ⅱ的表达与临床病理特征及无疾病生存期(DFS)的关系。用Ki-67标记细胞增殖指数,用TUNEL法检测细胞凋亡,并进行相关的统计学分析。结果胃腺癌HK-Ⅱ和TS阳性表达率分别为71.0%(44/62)和61.3%(38/62),且两者呈正相关关系。胃腺癌组织中HK-Ⅱ的高表达与肿瘤分化程度、淋巴结转移、临床分期有关及DFS短有关(P<0.05),但与患者年龄、性别和肿瘤部位无关(P>0.05)。与HK-Ⅱ阴性表达的患者相比,HK-Ⅱ阳性表达的胃腺癌组织增殖指数明显增高,凋亡指数显著降低(P<0.01)。结论胃腺癌组织中HK-Ⅱ的表达上调,其表达水平与肿瘤的分化、淋巴结转移、临床分期及预后有关,可能与化疗敏感性有关。HK-Ⅱ的高表达可能通过促进细胞增殖、诱导细胞凋亡,在胃癌的发生、发展及耐药过程中发挥重要作用。 ObjectiveTo investigate the expression of Hexokinase typeⅡ(HK-Ⅱ) protein in gastric adenocarcinoma and its relationship with proliferation, apoptosis and chemotherapy related protein (thymidylate synthase, TS).Methods Immunohistochemistry for HK-Ⅱand TS expression, Ki-67 (proliferative index) and TUNEL (apoptosis index) were examined in 62 gastric adenocarcinoma patients. ResultsThe positive rate of HK-Ⅱ expression and TS expression were 71.0% (44/62) and 61.3% (38/62), respectively, and the expression of HK-Ⅱ was positively correlated with TS expression in gastric adenocarcinoma. Overexpression of HK-Ⅱ was significantly correlated with tumor differentiation, lymph node metastasis, tumor stage (P〈0.05) and shorter DFS. Compared with those with negative HK-Ⅱ expression, there were an increase in proliferation index and a decrease in apoptosis index significantly in gastric adenocarcinoma with positive HK-Ⅱ expression.Conclusion HK-Ⅱ is highly expression in gastric adenocarcinoma, which may contribute to tumorigenesis, development and chemotherapy resistance through promoting proliferation and suppressing apoptosis.
