左西孟旦在心外科围术期的心脏保护作用

左西孟旦是一种钙离子增敏剂,可以增加心肌收缩力,扩张外周血管。左西孟旦起效迅速且作用持久,与其活性代谢产物OR-1896有关。可应用于心脏外科手术中保护心脏,减少缺血再灌注损伤,改善术后心脏功能,有效预防和治疗术后低心输出量综合症,提高术后脱离体外循环率。现有研究表明对于左室射血分数下降的患者左西孟旦能减少心脏术后死亡率,但仍需要一个大样本的临床随机对照试验来证明。

Levosimendan,a calcium sensitizer, can increase myocardial contractility and diastole peripheral vascular. It has a rapid onset of effect and long duration of action, which depends on its active metabolites OR - 1896. When applied to cardiac surgery, it can protect the heart from ischemia reperfusion injury and improve the postoperative cardiac function.And also,the prevention and treatment effect on the low cardiac output syndrome is obvious and the rate of weaned from extracorporeal circulation increased after operation. Existing studies have shown that levosimendan can reduce the postoperative mortality of patients with decreased left ventricular ejection fraction, but large sample randomized controlled clinical trials are still needed to get a more clear insight of it.
