严重下颈椎骨折脱位伴难复性关节突交锁手术治疗分析

Clinical treatment of sever lower cervical fracture dislocation and articular process interlocking by anteroposterior approach

目的：探讨1期后前联合入路手术治疗严重下颈椎骨折脱位伴难复性关节突绞锁的临床效果。方法：收集我院在2010年7月-2013年3月采用1期后前路联合手术进行治疗的22例严重下颈椎骨折脱位并关节突交锁患者，对其进行回顾性分析。采用美国脊髓损伤协会（ ASIA）神经功能评级对患者手术前后神经功能进行评价，观察手术治疗后患者神经功能恢复情况。结果：20例均获得随访，平均随访时间16个月，手术前后神经功能评级有显著差异（p＜0．05），患者神经功能均得到了一定程度的恢复。结论：1期后前路联合手术治疗严重下颈椎骨折脱位并难复性关节交锁，手术治疗效果满意。

Objective：To investigate surgical treatment of sever lower cervical fracture dislocation and articular process interlocking by anteroposterior approach and the clinical outcomes.Methods：Retrospective analysis was conducted on the clinical data of 22 patients with sever fracture and dislocation of lower cervical spine combined with locked facet,who were received from July 2010 to March 2013.The surgical outcome were evaluated with American Spinal Injury Association（ ASIA） retrospectively.Results：22 cases were followed up for 16 months in average.There are significant differences in scores of sensory and motor function before and after operation（ P〈0.05）.The postoperative follow-up of nerve function.obtained good reconstruction and maintenance.Conclusion： One-stage anteroposterior ap-proach had a satisfactory clinical effect in treating sever lower cervical fracture dislocation and irreducible articular process interlocking.
