前列腺泡沫状腺体型腺癌2例临床病理分析并文献复习

Clinicopathological analysis and literature review of two cases of prostatic foamy gland carcinoma

目的探讨前列腺泡沫状腺体型腺癌的临床病理特点、免疫组化特征及鉴别诊断,提高对前列腺泡沫状腺体型腺癌的诊治水平。方法结合文献复习,回顾性分析本院诊治的2例前列腺泡沫状腺体型腺癌患者临床资料。结果 2例均于泌尿外科B超引导下行前列腺细针穿刺活检术,镜检瘤细胞胞浆由于脂质大量堆积而成透明或泡沫状(黄色瘤样),核立方至柱状,小而深染,核仁不明显,但肿瘤的结构特征及生长方式与经典型癌相似,大多表现为密集排列的腺体和浸润性生长方式,是一种具有假良性细胞学表现的前列腺腺癌。结论泡沫状腺体型腺癌是前列腺腺泡腺癌的一种变异型,尽管细胞形态呈良性假象,但其生物学行为具有侵袭性。

Objective To investigate the clinicopathological characteristics, immunohistochemical features, differential diagnosis of foam gland adenocarcinoma of the prostate and promote the diagnosis and treatment. Methods The clinical data of 2 cases of prostatic foam gland carcinoma were analysed retrospectively. Results The needle biopsy under the guidance of ultrasonic showed that the cytoplasm was transparent and vacuolar because of lipid accumulation. The nucleus was small and hy-perchromatic, from cubic to column shape. The nucleolus was not obvious, but the structure and growth features, such as inten-sive gland and infiltrative growth patterns, were similar to classic adenocarcinoma. All the above suggested that the prostate ade-nocarcinoma may be with false benign cytological features. Conclusion The foam gland adenocarcinoma is a variant of pro-static acinar adenocarcinoma. Although the cell morphology is false benign , the biological behavior is usually aggressive.
