胰腺癌神经浸润途径的探讨和临床意义研究

目的对胰腺癌神经分布特点以及浸润途径进行分析探讨,并探究其临床意义。方法将35例发生癌变的胰腺以及8例正常的胰腺作为此次的研究对象,对胰腺进行大体解剖后,应用S-100免疫组化学染色法对胰腺进行染色处理,并用电镜进行观察,对两种胰腺的神经分布以及改变情况进行分析研究。结果胰头神经丛通常是来自右腹腔神经节、右腹腔肝丛以及腹腔丛右侧半,胰腺内神经在小叶间质内走形;胰腺内神经浸润阳性率为76.67%,胰腺外神经浸润阳性率为63.33%;胰腺癌神经浸润途径通常是通过直接对神经周膜进行破坏而侵入、进入神经周膜血管而侵入、对神经末梢突触膜进行破坏而侵入,浸润呈连续性。结论在进行胰头癌根治手术时,要对腹腔神经丛右侧半进行清扫。
