摘要
目的探讨突发性耳聋(sudden sensorineural hearing loss,SSHL)患者外周血白细胞计数及分属与初发听力损失程度和预后的相关性。方法 SSHL患者70例,常规进行血液学检查,记录外周血白细胞总数、中性粒细胞百分比、淋巴细胞百分比等项指标。分别采集治疗前后的纯音测听结果,观察白细胞计数和分属与患者初发听力损失程度和疗效的关系,以30例健康体检者的相关参数作为对照。结果SSHL组患者外周血白细胞计数、中性粒细胞百分比明显高于健康对照组,淋巴细胞百分比明显低于健康对照组(P<0.05);SSHL组患者白细胞计数和分属与初发听力损失程度无明显相关性(P>0.05),而外周血白细胞计数升高、中性粒细胞百分比升高及淋巴细胞百分比降低的患者治疗效果则明显较差(P<0.05)。结论 SSHL患者发病前可能存在感染因素,外周血白细胞计数和中性粒细胞百分比升高、淋巴细胞百分比降低可能提示患者预后不良。
Objective To explore the correlation of leukocyte count, genera in peripheral blood and the degree of incipient hearing loss, prognosis in patients with sudden sensorineural hearing loss (SSHL). Methods Seventy cases of SSHL patients and 30 cases of healthy controls were collected. Hematologic conventional examination was performed in all of the patients and healthy controls. The total number of leukocytes, percentage of neutrophil and lymphocyte were recorded. Meanwhile, the results of pure tone audiometry before and after treatment were respectively recorded. The relationship between leukocyte count and its genera and degree of hearing loss were analyzed. Results In SSHL group, peripheral leukocyte count and neutro- phil percentage phocytes was si were significantly higher than those in healthy control group ; percentage of lymgnificantly lower than that of healthy controls (P 〈 0. 05 ) ; leukocyte count and its genera in the patients with SSHL had no significant correlation with incipient degree of hear- ing loss (P 〉 0. 05). Treatment effect of the patients with elevated leukocyte count and neutro- phil percentage, or with decreased percentage of lymphocytes was obviously poor than those pa- tients with normal results of the blood test (P 〈 0. 05 ). Conclusion There may be infection before onset of the SSHL. Increase of leukocyte count and neutrophil percentage, or decrease of lymphocyte percentage may indicate a poor prognosis of the patients with SSHL.
