血清CA199、CA242及CA50水平联合检测对胰腺癌诊断的价值

Clinical Value of Combined Detection of CA199,CA242 and CA50 in Diagnosis of Pancreatic Cancer

目的探讨肿瘤标志物糖类抗原199(CA199)、CA242及CA50血清水平对胰腺癌的临床诊断价值。方法选择2012年1月至2013年10月深圳市宝安区人民医院收治的74例胰腺癌患者(胰腺癌组)和同期60例健康志愿者(健康对照组)采用化学发光法分别检测其血清CA199、CA242及CA50水平,分析对比CA199、CA242及CA50单一检测及联合检测的灵敏度、特异度及准确度。结果胰腺癌组肿瘤标志物CA199、CA242及CA50的血清水平均显著高于健康对照组[(20410±120)k U/L比(18±4)k U/L、(31201±246)k U/L比(679±90)k U/L、(58±15)k U/L比(14±4)k U/L(P<0.01)。3种肿瘤标志物的任意单一的灵敏度、特异度及准确度均低于三者联合的水平。结论肿瘤标志物CA199、CA242及CA50联合检测可提高对胰腺癌诊断的灵敏度、特异度及准确度。

Objective To discuss the clinical diagnosis value of tumor marker carbohydrate antigen 199（ CA199）,CA242 and CA50 serum level for pancreatic cancer. Methods A total of 74 pancreatic cancer patients（ pancreatic cancer group） admitted to Shenzhen City Baoan District People＇ s Hospital from Jan.2012 to Oct. 2013 were included in the study,and another 60 healthy volunteers（ healthy control group） during the same period were also included,chemiluminescence method was used for the detection of CA199,CA242 and CA50 levels,and comparative analysis of sensitivity,specificity and accuracy of serum CA199,CA242 and CA50 single testing and joint detection was done. Results Serum levels of tumor markers CA199,CA242 and CA50 of pancreatic cancer group were all significantly higher than the healthy control group [（ 204140 ± 120） k U / L vs（ 18 ± 4） k U / L,（ 31201 ± 246） k U / L vs（ 679 ± 90） k U / L,（ 58 ±15） k U / L vs（ 14 ± 4） k U / L,P 〈0. 01]. The sensitivity,specificity and accuracy of single test of any tumor markers were lower than the combined. Conclusion Tumor markers CA199,CA242 and CA50 combined detection can improve the sensitivity,specificity and accuracy of the diagnosis of pancreatic cancer.
