无创呼吸机辅助治疗急性左心衰竭对患者心率、血氧等因素的影响

Effects of noninvasive mechanical ventilation on heart rate, blood oxygen and other influenting factors of acute left heart failure

目的：观察无创呼吸机辅助呼吸治疗急性左心衰竭对患者的心率、呼吸、血压、PaO2、SpO2及患者的呼吸困难的症状缓解程度，探讨无创呼吸机的纠正急性左心衰患者的疗效。方法筛选2012年5月至2013年10月于义煤集团总医院接受治疗的80例急性左心衰竭患者作为观察对象，将所有患者随机分为对照组和实验组，每组各40例，对照组实行常规性药物治疗，实验组在常规药物治疗的基础上增加无创呼吸机辅助治疗，详细记录两组患者在治疗前后的心率、呼吸、血压、PaO2、SpO2等相关指标数据，并运用统计学软件进行处理分析。结果通过观察无创呼吸机治疗15 min及30 min后患者的心率、呼吸、血压、PaO2、SpO2及患者的呼吸困难的症状缓解程度，各项指标与对照治疗组比较具有统计学的意义（P〈0.05），实验治疗组的治疗效果优于对照组。结论在左心衰竭的治疗中使用无创呼吸机配合常规药物治疗效果显著，可提高常规药物治疗效果，可迅速减缓心率、降低血压、提高氧分压及血氧饱和度，同时可迅速缓解患者的呼吸困难症状。

Objective To observe the noninvasive ventilator breathing treatment in patients with acute left heart failure in heart rate, respiration, blood pressure, PaO2 , SpO2 , and the release of symptoms of respiratory difficulty, and thus investigate the curative effect of noninvasive ventilator on acute left heart failure. Methods Selected 80 cases of acute left heart failure patients from May 2012 to October 2012 as observation objects, and randomly divided all of the patients in-to control group and experiment group with 40 cases in each group. The control group received routine drug treatment and the experiment group was treated with noninvasive ventilator on the basis of conventional drug treatment. Heart rate, breath, blood pressure, PaO2 , SpO2 and other related index data were detailed recorded, and analyzed by using statistical software. Results By observing heart rate, respiration, blood pressure, PaO2 , SpO2 and patients with symptoms of respir-atory difficulty degree after the noninvasive ventilator treatment for 15 and 30 minutes after treatment. Compared with the control treatment group, the indicators in experiment group were significantly better （P〈0. 05）. Conclusions Noninva-sive ventilator combined with conventional drug treatment in the treatment of left heart failure is effective, and can improve the effect of conventional drug treatment and rapidly slow heart rate, lower blood pressure, improve the oxygen partial pres-sure and oxygen saturation, and can also rapidly relieve the patient’ s breathing difficulties.
