腹腔镜辅助胃癌根治术的近期及远期效果观察

Recent and long-term curative effect observation of gastric carcinoma treated by radical operation assisted by laparoscope

目的探讨腹腔镜辅助下实施胃癌根治术治疗的近、远期疗效。方法将2010年1月~2012年1月到我院普外科治疗的60例胃癌患者纳入研究,随机等分成两组,对照组采用传统开腹胃癌根治术救治,观察组实施腹腔镜辅助胃癌根治术救治,研究两组的淋巴结清除数、手术时长、术中失血量、术后肛门排气时间、恢复流质饮食时间、住院时间、合并症及3年无瘤存活情况等。结果两组实施治疗后的淋巴结清除数、3年无瘤存活率和平均存活时间均基本接近（P〉0.05）,观察组较对照组术中失血量少,术后肛门排气时间、恢复流质饮食时间、住院时间短,合并症发生率低（P〈0.05）。结论采用腹腔镜辅助胃癌根治术对胃癌患者进行救治,手术近期疗效更理想、安全,与开腹手术有相当的远期效果。

Objective To study the recent and long-term curative effect observation of gastric carcinoma treated by radical operation assisted by laparoscope. Methods 60 patients with gastric cancer treated in the general surgery de- partment of our hospital from January 2010 to January 2012 were studied and randomly divided into two groups.The control group was treated with laparotomy radical operation,and the observation group was treated with radical operation assisted by laparoscope.Curative effects including the number of clearing lymph node,the operation duration,the blood loss volume,the time of exsuffiation through anus after operation,recovery time of liquid diet,hospital stay,the complica- tion and the survival rate of without tumor in 3 years were studied. Results The number of clearing lymph node,the sur- vival rate of without tumor in 3 years and the average survival time between two groups was close respectively （P〉0.05）, but the blood loss volume,the time of exsufflation through anus after operation,hospital stay and the complication rate in observation group was less than the control group respectively（P〈0.05）. Conclusion The recent curative effect of gastric carcinoma patients treated by radical operation assisted by laparoscope is good and safe;the long-term curative effect is similar to the laparotomy radical operation.
