腹腔镜下结肠癌根治术与开腹手术治疗结肠癌的效果对比

Effect comparison of laparoscopic colon cancer radical surgery and la-parotomy in treatment of colon cancer

目的比较腹腔镜下结肠癌根治术与开腹结肠癌根治术治疗结肠癌的效果。方法选取2013年1月~2014年12月我院收治的88例结肠癌患者,根据入院编号随机分为观察组和对照组,各44例。对照组采用开腹结肠癌根治术方案,观察组则在腹腔镜下行结肠癌根治术,对比两组患者的术中出血量、腹腔引流量、手术时间、住院时间以及肛门排气时间等手术指标,并统计术后并发症发生情况。结果观察组患者的术中出血量、腹腔引流量为（89.5±15.5）、（281.4±92.6）ml,均显著少于对照组,手术时间、住院时间、肛门排气时间分别为（141.5±25.4）min、（8.5±2.2）d、（42.3±10.4）h,均显著短于对照组,差异有统计学意义（P〈0.05）;观察组患者术后发生吻合口出血2例,并发症发生率为4.5%,显著低于对照组的20.4%,差异有统计学意义（P〈0.05）。结论腹腔镜下结肠癌根治术治疗结肠癌的效果确切,具有创伤小、恢复快、并发症少的优势,值得在临床上推广和应用。

Objective To compare the effect of laparoscopic colon cancer radical surgery and laparotomic colon cancer radical surgery in the treatment of colon cancer. Methods Eighty-eight patients with colon cancer treated in our hospi- tal from January 2013 to December 2014 were selected and randomly divided into observation group and control group according to the hospital admission number,and there were patients in each group.Control group was given laparotomic colon cancer radical surgery and observation group was given laparoscopic colon cancer radical surgery.Surgical indexes such as intraoperative bleeding amount,peritoneal drainage amount,operative time,hospital stay and anal exhaust time between two groups was compared respectively,and the occurrence of postoperative complications were counted. Results The intraoperative bleeding amount, peritoneal drainage amount [（89.5_＋15.5） m1,（281.4＋92.6） ml respectively] was obvi- ously less than that of control group and operative time,hospital stay and anal exhaust time of observation group [（141.5＋25.4） rnin,（8.5~2.2） d and （42.3_＋10.4） h respectively] was significantly lower than that of control group,and there was a statistical difference （P〈0.05）.Postoperative anastomotic stoma bleeding occurred in 2 patients of observa- tion group,and the incidence rate of complication （4.5%） was significantly lower than that of control group （20.4%）,and there was a statistical difference （P〈0.05）. Conclusion Effect of laparoscopic colon cancer radical surgery in the treat- ment of colon cancer is exact,and it has the advantages of small incision,fast recovery and fewer complication,which is worthy of clinical promotion and application.
