经侧裂入路手术治疗早期高血压基底节区脑出血的效果观察 被引量:3

摘要 目的 探讨经侧裂入路手术治疗早期高血压基底节区脑出血的临床效果。方法 选取2013年1月~2014年6月本院收治的79例早期高血压脑出血患者作为研究对象,随机分为观察组(54例)和对照组(25例)。观察组采用经侧裂入路手术,对照组采用传统手术,比较两组的治疗效果。结果 观察组的血肿清除率显著高于对照组,再出血率显著低于对照组。差异有统计学意义(P〈0.05)。观察组术后6个月的ADL功能评分显著优于对照组,差异有统计学意义(P〈0.05)。观察组的死亡率显著低于对照组,差异有统计学意义(P〈0.05)。结论 经侧裂入路手术治疗高血压基底节区脑出血的血肿清除率高,患者的术后恢复较好,对于降低死亡率、改善患者生存质量具有重要意义。 Objective To explore the clinical effect of transsylvian approach surgery in the treatment of early hypertensive cerebral hemorrhage in basal ganglia region. Methods 79 patients with early hypertensive cerebral hemorrhage in basal ganglia region from January 2013 to June 2014 in our hospital were selected and randomly divided into the ob- servation group (n=54) and the control group (n=25).The observation group was treated with transsylvian approach oper- ation,the control was treated with traditional operation.The clinical effect in two groups was compared. Results The clearance rate of hematoma in the observation group was higher than that in the control group,the rate of rehaemorrha- gia was lower than that in the control group,with significant difference (P〈0.05).The functional score of activity of daily living (ADL) in the observation group six months after surgery was better than that in the control group,with significant difference (P〈0.05).The mortality in the observation group was lower than that in the control group,with significant difference (/9〈0.05). Conclusion Transsylvian-insular approach in the surgical treatment of early hypertensive cerebral hemorrhage in basal ganglia region has a high clearance rate of hematoma,good postoperative recovery,and which is of great significance of decreasing mortality of hypertensive cerebral hemorrhage in basal ganglia region and improving patient's quality of life.
