荆防颗粒治疗扁平疣的临床疗效及作用机制研究

Clinical Effects and Mechanism of Jingfangtang Granules on Patients with Verruca Plana

目的:评价荆防颗粒内服治疗扁平疣的临床疗效和安全性。方法:110例扁平疣患者随机分为治疗组和对照组。治疗组采用口服荆防汤颗粒剂,2次/d;对照组给予0.1%维A酸乳膏涂抹患处,1次/d,疗程均为8周。观察皮损扁平疣数量、局部不良反应。对治愈病例随访1个月,观察复发情况。采用流式细胞技术检测患者外周血T淋巴细胞亚群,采用ELISA法检测外周血IL-2、IFN-γ和TNF-α含量,采用荧光定量PCR技术检测皮损组织TLR4和COX2相对表达量。结果:治疗组有效率为87.3%,而对照组有效率为61.8%。两组经Ridit分析比较:χ2=8.10。治愈病例随访1个月,对照组15例,复发8例,复发率53.3%;治疗组21例,复发2例,复发率9.5%,复发率低于对照组(P<0.01)。治疗组外周血T淋巴细胞亚群中CD+3、CD+4、CD+4/CD+8和IL-2、IFN-γ含量均较治疗前明显升高,而CD+8和TNF-α则明显降低。同时,治疗组皮损组织较TLR4和COX2相对表达量较治疗前明显降低。两组均未见明显不良反应。结论:荆防颗粒内服治疗扁平疣具有一定的临床疗效和安全性。

Objective ：To investigate the clinical effects and safety of Jingfan granules on the patients with verruca plana. Methods ： One hundred and ten cases of patients with verruca plana were randomly divided into treatment group and control group. The patients in the treatment group were orally administrated with Jingfan granules twice a day. The control group was given 0. 1% tretinoin cream daily on infected part. Both groups were treated for 8 weeks. T - lymphocyte subgroups in peripheral blood of all the patients were determined by flow cytometry while IL - 2, IFN - γ and TNF - α by ELISA. Results：Compared with effective rate as 61. 8% in control group, it was 83.7% in treatment group. The difference between two groups was significant according to the result of Ridit analysis （ X2 = 8.10,P 〈 0. 01 ）. After a month＇ s follow - up,the relapse rate of control group was 53.3% while the treatment group＇ s was 9.5%. No adverse reaction was found in both groups. After the treatment of transferring factor, increase of CD3＋ cells, CD4＋ cells, CD4＋/CD8＋ ratio, IL - 2 and IFN -γand decrease of CD8＋ cells and TNF -α levels were found in the treatment groups as compared with those before treatment. The expressions of TLR9 protein in skin lesions were also down - regulated compared with that in the control. Conclusion Jingfan granule was effective and safe on patients with verruca plana by oral administration.
