The Alberta Stroke Program Early CT Score (ASPECTS) is a simple, reliable and systematic approach for evaluating early ischemic changes in the middle cerebral artery territory in patients with ischemic stroke. ASPECTS is helpful in predicting thrombolytic effect and long-term prognosis. The developments of CTA, multimodal CT and MRI techniques and the posterior circulation ASPECTS have enriched the application scope of ASPECTS and have improved the reliability of disease judgment. In this article we reviewed the ASPECTS, its reliability, different models and applications in diagnosis, therapy and prognosis of acute stroke.
