个体化护理对ICU重症肺炎患者生理指标改善和预后效果的影响

Influences of individual care on the physiological indexes improvement and prognosis effects of ICU severe pneumonia patients

目的：探讨个体化护理方案对ICU重症肺炎患者生理指标的改善情况,以及对预后效果的影响。方法将2008年3月—2014年3月收治的94例重症肺炎患者按入住ICU顺序随机分为对照组和观察组,分别采取常规护理和个体化护理,比较两组血乳酸水平、乳酸清除率、急性生理学和慢性健康评估系统（APACHEⅡ）评分等生理指标以及出院6,12个月后的预后效果。结果干预后观察组乳酸水平低于对照组,乳酸清除率高于对照组,差异有统计学意义（P〈0.05）；干预后观察组APACHEⅡ评分和预测病死率均低于对照组,差异有统计学意义（P〈0.05）。出院6,12个月后观察组患者活动受限、心绞痛发作、生活质量评分均优于对照组,差异有统计学意义（P〈0.05）。结论个体化护理服务能够改善患者血乳酸和APACHEⅡ评分等生理指标,提高患者预后的生活质量,有利于患者的尽快康复。

Objective To explore the influences of individual care on the physiological indexes improvement and prognosis effects of ICU severe pneumonia patients. Methods There were 94 sever pneumonia patients were divided into control group and observation group by the sequence of ICU admission from March 2008 to March 2014. We compared lactic acid （LAC）, lactate clearance rate, acute physiology, APACHE Ⅱand so on other physiological indexes, and prognosis effects 6 and 12 months after discharge. Results After intervention, LAC of the observation group was lower than that of the control group （P 〈0. 05）, but lactate clearance rate of the observation group was higher compared with that of the control group （P〈0. 05）. After the intervention,the APACHEⅡscores and predicted mortality in the observation group were lower than those in the control group（P〈0. 05）. After 6 and 12 months′intervention in the observation group, the scores of restricted activities, angina pectoris attacks, and quality of life were better than those of the control group （P〈0. 05）. Conclusions The individual care can contribute to patients′ physiological indexes of LAC and APACHE Ⅱ, improve the prognosis quality of life, and promote patients to recover.
