加巴喷丁联合窄谱中波紫外线治疗老年带状疱疹的临床疗效

Effect of gabapentin combined with narrow-band ultraviolet B on herpes zoster in the elderly

目的观察加巴喷丁联合窄谱中波紫外线(narrow-band ultraviolet B,NB-UVB)照射治疗老年带状疱疹的临床疗效及安全性。方法 90例老年带状疱疹患者随机分为A、B、C 3组:3组患者常规口服伐昔洛韦片抗病毒治疗,在此基础上,A组:口服加巴喷丁;B组:NB-UVB照射治疗;C组:口服加巴喷丁联合NB-UVB照射治疗;观察3组疗效。结果 C组水疱结痂、脱痂时间均明显短于A组(P<0.05),疼痛缓解、疼痛消失时间均明显短于B组(P<0.05),疼痛消失时间亦明显短于A组(P<0.05);C组有效率显著优于A、B组(P<0.05),带状疱疹后遗神经痛(post-herpetic neuralgia,PHN)发生率明显低于A、B组(P<0.05)。结论加巴喷丁联合NBUVB照射治疗老年带状疱疹的疗效优于单用加巴喷丁或NB-UVB治疗,并可减少PHN的发生率,是治疗老年带状疱疹安全有效的方法。

Objective To observe the effect and safety of gabapentin combined with narrow-band ultraviolet B（ NBUVB） on herpes zoster in the elderly. Methods Ninety elderly patients with herpes zoster were randomly divided into three groups. Patients in group A were treated with gabapentin,group B treated with NB-UVB radiation and group C treated with a combination of gabapentin and NB-UVB. All patients in three groups were administrated with valaciclovir. Then the patients were followed up and the therapeutic effects were observed. Results The blister scab time and decrustation time of group C were obviously shorter than those of group A（ P 〈 0. 05）. The pain relief time and pain vanishing time of group C were significantly shorter than those of group B（ P 〈 0. 05）. The pain vanishing time of group C was also obviously shorter than that of group A（ P 〈 0. 05）. In addition,the effective rate of group C was higher than that in both group A and group B（ P 〈 0. 05）. However,there was no significant difference between group A and group B in effective rate（ P 〉0. 05）. The rate of post-herpetic neuralgia（ PHN） in group C was significantly lower than that of group A and group B（ P 〈 0. 05）. Conclusions Combined therapy of gabapentin and NB-UVB irradiation is an optimal choice for the treatment of herpes zoster in the elderly. It is effective,safe and can reduce PHN rate.
