妇科恶性肿瘤围术期预防下肢深静脉血栓的治疗研究

A TREATMENT RESEARCH ON PREVENTION OF LOWER EXTREMITY DEEP VEIN THROMBOSIS IN THE GYNECOLOGICAL MALIGNANT TUMOR PERIOPERATION

目的观察和分析减少妇科恶性肿瘤患者围术期深静脉血栓的发生及其预防治疗。方法选取2011年8月—2013年8月该院收治的76例妇科恶性肿瘤患者,其中宫颈癌患者26例,子宫内膜癌患者30例,卵巢上皮性癌患者20例。按照是否采用预防治疗方式分为二组,其中观察组38例妇科恶性肿瘤患者采取了预防下肢深静脉血栓治疗,另外38例妇科恶性肿瘤患者没有采取预防血栓治疗为对照组。在术前、术中、术后采取预防措施和早期治疗,判断下肢深静脉血栓及肺栓塞形成的几率。结果治疗组38例妇科恶性肿瘤患者只有1例在手术后形成深静脉血栓,采取有效治疗后均治愈,无一例发生肺栓塞。对照组38例未采取预防血栓治疗,14例患者出现深静脉血栓,2例出现肺栓塞,观察组明显好于对照组(P<0.05),差异有统计学意义。结论通过对妇科恶性肿瘤围术期对下肢深静脉血栓的评估,强化风险意识教育,积极妥善地进行术前术后治疗,可明显降低围术期下肢深静脉血栓形成几率。

Objective To observe and analyze reducing perioperative patients with gynecological malignant tumor and its prevention and treatment of deep vein thrombosis. Methods We selected 76 cases of gynecologic malignant tumor patients as study subjects in August 2011 to August 2013 in author＇s hospital, 26 cases of cervical cancer patients, 30 patients with endometrial carcinoma, 20 patients with epithelial ovarian carcinoma. They were divided into two groups according to whether to adopt preventive treatments, the treatment group 38 cases of gynecologic malignant tumor patients treated with the prevention of lower extremity deep vein thrombosis, and 38 patients with gynecological malignant tumor treatment for the control group did not take preventive a blood clot. In the preoperative, intraoperative and postoperative, preven- tion measures and early treatment were to judge the probability of lower extremity deep vein thrombosis and pulmonary embolism formation. Results There was only 1 case in the formation of deep venous thrombosis after the operation in the treatment group （38 cases of gynecologic malignant tumors）, and was cured after effective treatment, no case of pulmonary embolism. Control group 38 cases of failure to prevent blood clots, deep vein thrombosis was observed in 14 patients, 2 cases of pulmonary embolism, the effect in the treatment group was significantly better than the control group （P〈0.05）. Conclusion It can obviously reduce the perioperative risk lower extremity deep vein thrombosis of gynecologic malignant tumor perioperative patients to strengthen evaluation of lower extremity deep vein thrombosis, risk consciousness education, preoperative and postoperative therapy.
