2型糖尿病肥胖患者中肿瘤坏死因子-α的水平及意义 被引量:1

摘要 目的研究肥胖的2型糖尿病患者血清中肿瘤坏死因子-α(TNF-α)的水平和意义;以及TNF-α与空腹血糖(FBG)、收缩压(SBP)、舒张压(DBP)、腰臀比(WHR)、体重指数(BMI)、甘油三酯(TG)、胆固醇(CHO)的关系。方法随机选择肥胖患者68例(男33例,女35例),平均年龄(47.55±8.21)岁,其中包括T2DM患者38例(男17例,女21例),平均年龄(51.89±10.64)岁及单纯肥胖(simple obesity,SO组)患者30例(男21例,女14例),平均年龄(48.55±8.21)岁。并随机选择体重正常的正常对照(NC)22例(男12例,女10例),平均年龄(48.24±6.28)岁。所有受试者均排除炎症、心血管病变及肿瘤。采用酶联免疫吸附试验(Enzyme Linked Immunosorbent Assay,ELISA)的方法测定血清中TNF-α的水平,同时计算BMI、WHR,并测定FBG。三组间各指标比较用单因素方差分析;三组之间两两比较用LSD-t检验;各指标间的相关性用直线相关分析和多元逐步回归分析。结果 1三组间的WHR差异有统计学意义(F=11.092,P<0.05),其中T2DM组及单纯肥胖组WHR高于及NC组(P<0.05),T2DM组及单纯肥胖组WHR无统计学意义(P>0.05);2T2DM组、单纯肥胖组和NC组TNF-α差异有统计学意义(P<0.05);3三组之间两两比较,单纯肥胖组高于NC组而低于T2DM组,差异有统计学意义(P<0.05);4TNF-α水平与FBG呈正相关(r=0.257,P<0.05)。结论 TNF-α水平与FBG、BMI、WHR呈正相关,且FBG是TNF-α的独立相关因素。2型糖尿病患者及糖调节正常的单纯肥胖患者血清TNF-α水平增高,可能是机体针对体内血糖增高、胰岛功能受损所发生的一种代偿效应,提示TNF-α可能是与糖尿病和肥胖,尤其是腹型肥胖有关的脂肪细胞因子。
