胰岛素联合盐酸二甲双胍治疗2型糖尿病疗效观察

Observation of Curative Effect of Insulin Combined With Metformin Hy-drochloride in the Treatment of Type 2 Diabetic Patients.

目的探讨胰岛素联合盐酸二甲双胍治疗2型糖尿病的临床效果。方法选择2011年3月—2014年10月在该院治疗并同意配合观察的80例病人。随机分为对照组和观察组,各40例。两组患者在治疗前均给予糖尿病基础教育包括:饮食治疗、运动治疗、自测血糖、用药安全等。对照组单用胰岛素诺和灵30R皮下注射,观察组给予诺和灵30R+盐酸二甲双胍口服联合治疗。结果两组患者治疗后空腹血糖(FBG)、餐后2Hp G、糖化血红蛋白(Hb A1C)、胰岛素用量以及血脂、体重指数等均有下降(P<0.05),但观察组各项指标均较对照组下降明显,两组对比差异有统计学意义(P<0.05)。结论胰岛素联合盐酸二甲双胍治疗2型糖尿病,改善胰岛素敏感性,减少胰岛素用量同时减少了不良反应的发生,不但能有效控制血糖;安全方便,患者依从性好,特别适用于基层临床推广应用。

Objective To investigate the clinical effect of insulin combined with metformin in the treatment of type 2 dia-betes mellitus. Methods 80 cases of type 2 diabetic patients were selected who were treated in our hospital from 2011 March to 2014 October and agreed to cooperate with the observation and were randomly divided into the control group and the observation group each of 40 cases. Before the treatment, two groups were educated including： diet therapy, exercise therapy, self-monitoring, medication safety et al. The control group was treated with subcutaneous insulin Novolin 30R, ob-servation group was treated with Novolin 30R ＋ metformin hydrochloride oral combination therapy. Results FBG, 2hPG, HbA1c, insulin dosage, serum lipid and BMI of observation group were lower than the control groups, and the difference was statistically significant（P〈0.05）. Conclusion Insulin combined with metformin hydrochloride treatment of type 2 diabetic mellitus was helpful to improve insulin sensitivity, reduce insulin dosage and adverse reaction but not good to control blood sugar levels. It would be the ideal method for treating type 2 diabetic patients, considering the general safety effects and convenience. Especially, it was suitable for primary clinical application.
