补中益气汤辨证治疗合并脑梗死伴随神经病变糖尿病患者效果的临床探讨

Clinical Discussion of Buzhong Yiqi Decoction Combined with Cerebral Infarction Patients with Syndrome Differentiation and Treatment of Diabetic Neuropathy

目的探讨常规治疗联合补中益气汤辨证治疗合并脑梗死伴有神经病变糖尿病患者的临床效果。方法选择近年来该院经治的90例合并脑梗死伴有神经病变糖尿病患者为研究对象,按照随机化原则分为对照组和研究组各45例,两组患者均给予控制血糖(应用口服降糖药,必要时给予胰岛素)、饮食治疗和早期康复治疗,在此基础上对照组给予静点川芎嗪加用维生素B1、甲钴胺的常规治疗方法,研究组辨证加减合理应用补中益气汤,两组均治疗30 d,观察患者治疗前后临床症状改变和神经功能缺损评分,测定神经传导速度,评价两组患者治疗效果之间的差异是否具有统计学意义。结果研究组45例患者中,显效29例,有效12例,无效4例,总有效率为91.11%;对照组45例患者中,显效、有效和无效的人数分别为16例、11例和18例,总有效率为60.00%,两组患者疗效和总有效率之间的差异有统计学意义(P<0.05),即研究组优(高)于对照组。结论在良好控制血糖、饮食治疗和早期康复治疗的基础上,应用补中益气汤辨证治疗合并脑梗死伴有周围神经病变糖尿病患者的疗效较为满意。

Objective To discuss the clinical effect of conventional treatment combined with Buzhong Yiqi Decoction syndrome differentiation in the treatment of patients with cerebral infarction accompanied with neural lesions in diabetic patients. Methods In our hospital treated 90 cases of cerebral infarction complicated with diabetic neuropathy patients as the research object, randomly divided into control group and study group with 45 cases in each group, two groups of patients were given control of blood glucose （Application of oral hypoglycemic drugs, the need for insulin）, diet therapy and early rehabilitation treatment, on the basis of the control group was given intravenous Ligustrazine plus conventional treatment with vitamin B1, methylcobalamin, reasonable application of Buzhong Yiqi Decoction group of dialectical addition and subtraction, two groups were treated for 30d, and the change of neural function defect score of clinical symptoms were observed before and after treatment, measurement of nerve conduction velocity, whether the difference between the treatment effect of two groups were statistically evaluated meaning. Results In the study group, 45 cases were markedly effective, 12 cases were effective, 4 cases were ineffective, the total effective rate was 91.11%. 45 cases of control group were 16 cases, 11 cases and 18 cases, respectively. The total effective rate was 60%. The difference between the two groups were significant （P〈0.05）. Conclusion In good control blood sugar, diet therapy and early rehabilitation treatment based. Effect of application of Buzhong Yiqi Decoction syndrome differentiation in the treatment of patients with cerebral infarction accompanied with peripheral neuropathy in diabetic patients is satisfactory.
