基质金属蛋白酶-9在颈动脉斑块易损性中的诊断价值

The Diagnostic Value of Matrix Metalloproteinase-9 in the Vulnerability of Carotid Atherosclerotic Plaque

摘要 目的通过二维超声对颈动脉斑块进行分型并联合测定患者血浆基质金属蛋白酶-9(MMP-9)的水平,探讨MMP-9在缺血性脑梗死患者颈动脉粥样硬化斑块易损性中的诊断价值。方法 90例缺血性脑梗死合并颈动脉粥样斑块患者根据二维超声颈动脉粥样硬化斑块特点分为易损性组(48例)及非易损性组(42例);同时随机选取我院体检中心健康者40例作为对照组;分别测定3组患者血浆MMP-9的水平。结果脑梗死伴颈动脉硬化组MMP-9水平明显高于对照组(P<0.01);易损性组MMP-9水平明显高于非易损组(P<0.01);血浆MMP-9水平与斑块易损性呈正相关(r=0.89,P<0.01)。结论血清MMP-9是评估缺血性脑梗死患者颈动脉粥样硬化斑块易损性的一项有效、敏感及非侵入性生物学标志物,与二维超声结合可以作为评估缺血性脑梗死危险因素的辅助实验室检查指标。 Objective To expIore the reIationship between the vuInerabiIity of carotid atheroscIe-rotic pIaques in patients with ischemic stroke by two-DimensionaI uItrasound of carotid artery pIaque type and IeveI of joint determination of pIasma matrix metaIIoproteinase-9 (MMP-9). Methods 90 cases of patients with ischemic stroke were randomIy seIected in NeuroIogy, de-pending on the nature of atheroscIerotic pIaques into vuInerabiIity group ( 48 cases) and non-vuInerabIe pIaque group ( 42 cases);another normaI 40 cases in our hospitaI medicaI randomIy seIected center as a controI group;MMP-9 IeveIs were measured in three groups,and correIa-tion anaIysis was made.Results Two-dimensionaI uItrasound can dispIay the stabiIity of carotid atheroscIerotic pIaque.CerebraI infarction with carotid atheroscIerosis group MMP-9 IeveI was significantIy higher (P〈0.01);VuInerabIe pIaque group MMP-9 IeveIs were significantIy higher than those of non-vuInerabIe pIaque group ( P〈0.01 );reIevant ( r=0.89, P〈0.01 ) in patients with ischemic cerebraI infarction between MMP-9 IeveIs and vuInerabiIity of carotid atheroscIe-rotic pIaques.Conclusion MMP-9 is an effective,sensitive and non-invasive biomarkers to as-sess the vuInerabiIity of ischemic stroke carotid atheroscIerotic pIaques.It can aIso be used as an auxiIiary Iaboratory parameters used to assess ischemic stroke risk factors when combined with the two-dimensionaI uItrasound.
