摘要
目的 探讨动脉弹性与心血管危险因素的相关性.方法 纳入2013年5月至2014年10月北京老年医院心内科体检及门诊病例181例,以颈-股动脉脉搏波速度(cfPWV) 10 m/s为界值分为cfPWV正常组(<10 m/s,87例)和cfPWV升高组(≥10 m/s,94例).记录受试者年龄、腰围、体重指数,检测血压、血糖、血脂、胰岛素、胰岛素抵抗指数(HOMA-IR)、高敏C反应蛋白(hs-CRP)、内皮素、一氧化氮水平及颈-桡动脉脉搏波速度(crPWV).分析cfPWV和crPWV与上述因素的相关性.结果 cfPWV升高组与cfPWV正常组腰围、收缩压、舒张压、三酰甘油、血糖、hs-CRP、HOMA-IR、内皮素、一氧化氮和crPWV比较差异均有统计学意义[(85±9)cm比(81±11)cm,(139±19)mmHg(1 mmHg =0.133 kPa)比(121±17) mmHg,(91±12) mmHg比(82± 13)mmHg,(2.5± 1.3)mmol/L比(1.6±1.0) mmol/L,(5.8±1.6) mmol/L比(5.2 ±0.5) mmol/L,(2.8±1.4) mg/L比(1.6 ±0.6) mg/L,(5.9±2.8)比(4.0±2.1),(81±21) ng/L比(74±21) ng/L,(43±21) μmol/L比(56±25) μmol/L,(10.3±.5) m/s比(9.0±1.5)m/s](均P<0.05).Pearson相关分析表明,cfPWV与年龄、收缩压、舒张压、血糖、腰围、三酰甘油、总胆固醇、内皮素、一氧化氮、HOMA-IR和hs-CRP具有相关性(均P<0.05);crPWV与年龄、收缩压、舒张压、一氧化氮具有相关性(均P<0.05).多元逐步线性回归分析表明收缩压、年龄、血糖、体重指数、总胆固醇和hs-CRP是cfPWV的独立影响因素(均P<0.05);收缩压、舒张压、体重指数、年龄、腰围和一氧化氮是crPWV的独立影响因素(均P<0.05).结论 年龄、血压、血糖、腰围、体重指数、总胆固醇、hs-CRP和一氧化氮是动脉弹性的独立影响因素.
Objective To investigate the correlations of arterial elasticity and cardiovascular risk factors.Methods Totally 181 subjects from May 2013 to October 2014 were enrolled and divided into normal carotid-femoral artery PWV (cfPWV) group (〈 10 m/s,87 subjects) and increased cfPWV group (≥ 10 m/s,94 subjects).Age,waistline and body mass index (BMI) were recorded;blood pressure,blood glucose (BG),blood lipid,insulin,insulin resistance (HOMA-IR),high sensitivity C-reactive protein (hs-CRP),endothelins (ET),nitricoxide (NO) and catroid-radial artery PWV (crPWV) were measured.The correlations of cfPWV,crPWV with above factors were analyzed.Results The waistline,systolic blood pressure (SBP),diastolic blood pressure (DBP),triglyeride (TG),BG,hs-CRP,HOMA-IR,ET,NO and crPWV in increased cfPWV group were significantly different from those in normal cfPWV group [(85 ± 9) cm vs (81 ± 11) cm,(139±19) mmHgvs (121 ±17) mmHg,(91 ±12) mmHg vs (82±13) mmHg,(2.5 ±1.3) mmol/L vs (1.6± 1.0) mmol/L,(5.8 ± 1.6) mmol/L vs (5.2 ±0.5) mmol/L,(2.8 ± 1.4) mg/L vs (1.6 ±0.6) mg/L,(5.9 ±2.8) vs (4.0 ±2.1),(81 ±21) ng/L vs (74 ±21) ng/L,(43 ±21) μmol/L vs (56 ±25) μmol/L,(10.3 ±1.5) m/s vs (9.0 ±1.5) m/s] (all P 〈 0.05).Pearson correlation analysis showed that cfPWV was correlated with SBP,age,DBP,BG,TG,waistline,total cholesterol (TC),HOMA-IR,ET,NO and hs-CRP (P 〈 0.05);crPWV was correlated with DBP,age,SBP and NO (P 〈0.05).Multiple linear stepwise regression showed that SBP,age,BMI,TC and hs-CRP were independent factors of cfPWV (P 〈 0.05);SBP,DBP,BMI,age,waistline and NO were independent factors of crPWV (P 〈0.05).Conclusion Age,blood pressure,blood glucose,waistline,BMI,TC and hs-CRP and NO are independent factors of arterial elasticity.
